IntroductionLe syndrome métabolique est associé aux maladies cardiovasculaires. L'infection au VIH est devenue aujourd'hui une maladie chronique. L'objectif de cette étude est de déterminer la prévalence, les facteurs associés et prédisposant au syndrome métabolique chez les patients infectés par le VIH sous traitement antirétroviral.MéthodesIl s'est agi d'une étude transversale, descriptive et analytique. La population d’étude est constituée des patients vivant avec le VIH sous antirétroviral suivis au Centre Hospitalier Universitaire de l'Ouémé-Plateau. Le syndrome métabolique a été défini selon les critères de la Fédération Internationale du Diabète.RésultatsLa population était constituée de 244 patients. La prévalence du syndrome métabolique était de 18,03% avec une prédominance féminine (74,6%). La moyenne d’âge était de 40,7 ± 9,71 ans. Les facteurs associés au syndrome métabolique étaient le sexe féminin, la sédentarité, l'antécédent d'HTA, le surpoids, l'apport énergétique élevé, l'apport lipidique élevé, la consommation d'alcool, la consommation de tabac et l'hypercholestérolémie. Les facteurs prédisposant au syndrome métabolique étaient la présence de l'HTA, le tour de taille élevé, l'hyperglycémie, l'hypocholestérolémie HDL et l'hypertriglycéridémie.ConclusionLe syndrome métabolique est fréquent chez les patients infectés par le VIH sous traitement antirétroviral. Une prévention prenant en compte les facteurs associés et prédisposant s'avère nécessaire.

RESUMEAu regard des connaissances actuelles, aucune étude n'est disponible au Bénin sur le profil lipidique des personnes vivant avec le VIH. L'objectif de la présente étude est de décrire le profil lipidique des personnes vivant avec le VIH sous antirétroviral suivies au CHU-O/P de Porto-Novo. Il s'est agi d'une étude transversale, descriptive et analytique. La population d'étude a été constituée les personnes vivant avec le VIH sous antirétroviral suivies CHU-O/P de Porto-Novo. La collecte des données a été faite à la base d'une fiche de collecte chez les personnes éligibles et ayant donné leur consentement. Le profil lipidique a été apprécié par le bilan lipidique réalisé chez les patients à jeun. Au total 244 patients ont été inclus dans l'étude. On a noté une prédominance des femmes (74,6%). La moyenne d'âge était de 40,7 ± 9,71 ans. L'hypercholestérolémie totale a été observée chez 101 patients (41,39%), l'hypercholestérolémie LDL chez 84 patients (34,83%), l'hypocholestérolémie HDL chez 68 patients (27,87%) et l'hypertriglycéridémie chez 44 patients (18,03%). Le rapport triglycéridémie sur cholestérolémie HDL est élevé chez 45 patients (18,44%). Le surpoids a été associé aux dyslipidémies. The high total cholesterol was observed in 101 patients (41.39%), high LDL cholesterol in 84 patients (34.83%), low cholesterolemia HDL in 68 patients (27.87%) and high triglyceridemia in 44 patients (18.03%). The ratio triglycerides and HDL cholesterol is elevated in 45 patients (18.44%). Overweight was associated with dyslipidemia. The lipid profile of patients infected with HIV and on antiretroviral this study is characterized by the presence of dyslipidemia favored by overweight.

Background: A healthy diet is essential for optimal diabetes management. However, dietary habits vary from one region to another, making it challenging to standardize practices. Objective: To describe the diet habits of patients living with diabetes in Guinea. Methods: We conducted a dietary habits survey among 102 patients living with diabetes followed-up at the University Hospital of Conakry in Guinea between January and March 2012. Data were collected by individual interview on the basis of a three-item questionnaire: diabetes data, diet mode, and food composition. Results: In total, 85% of patients were consuming 3 meals daily; 13.7% had a collation and 25% had snacking habits. The meal was individual in 82.4% and collective in 17.6% of patients. The main foods consumed daily were: rice (93.1%), fish (93.1%), palm oil (91.1%), bread (87.2%). Foods consumed regularly (2 to 3 times a week) were: meat (49%), eggs (23.5%), dairy products (34.3%), fruit (43.1%), vegetables (40.2%) and peanut oil (21.5%). The food bans reported were: regular sugar (100%), sodas (62.7%), peanuts (84.3%) and sweetened fruits (55.8%). Conclusion: Combining dietary recommendations and dietary habits is essential for appropriate management of diabetic patients. The assessment of local food glycemic indexes and the training of dietitians remains a challenge in our context.

Background: To determine the prevalence of tuberculosis (TB) and associated factors in persons with diabetes mellitus (DM) in Benin, Guinea and Senegal.Methods: A cross-sectional study was conducted in the largest DM center in each country. Participants systematically underwent clinical screening and chest radiography. Participants who were symptomatic or with abnormal radiography underwent bacteriological investigations (sputum smear, Xpert MTB/RIF and culture) on sputum. Participants with no TB at enrolment were re-examined for TB six months later. Logistic regression was performed to identify factors associated with TB.Results: There were 5870 DM patients: 1881 (32.0%) in Benin, 1912 (32.6%) in Guinea and 2077 (35.4%) in Senegal. Of these, 114 had bacteriologically-confirmed TB, giving a pooled prevalence of 1.9% (95%CI=1.6-2.3). TB prevalence was 0.5% (95%CI=0.3-1.0), 2.4% (95%CI=1.8-3.2) and 2.8% (95%CI=2.2-3.6), respectively, in Benin, Guinea and Senegal. Factors associated with an increased odds of TB diagnosis were a usual residence in Guinea (aOR=2.62;95%CI=1.19-5.77; p=0.016) or in Senegal (aOR=3.73;95%CI=1.85-7.51; p<0.001), the age group of 35-49 years (aOR=2.30;95%CI=1.11-4.79; p=0.025), underweight (aOR=7.34;95%CI=4.65-11.57; p<0.001) and close contact with a TB case (aOR=2.27;95%CI=1.37-3.76; p=0.002). Obesity was associated with lower odds of TB (aOR=0.20; 95%CI=0.06-0.65; p=0.008).Conclusion: TB is prevalent among DM patients in Benin, Guinea and Senegal and higher than among the general population. The findings support the need for intensified case finding in DM patients in order to ensure systematic early detection of TB during the routine consultation process.
