<p class="p1"><strong>Antecedentes.</strong> La competencia para el cuidado en el hogar es parte de las características fundamentales requeridas en la atención de personas con enfermedad crónica y se describe como la capacidad, habilidad y preparación que tiene el usuario o su cuidador familiar para ejercer la labor de cuidar. </p><p class="p1"><strong>Objetivo.</strong> Describir y comparar la competencia para cuidar en el hogar de personas con enfermedad crónica, así como la competencia de sus cuidadores familiares para atenderlas en el hogar.</p><p class="p1"><strong>Materiales y métodos.</strong> Estudio cuantitativo, descriptivo, comparativo y de corte transversal desarrollado entre 2012 y 2014 en las cinco regiones geográficas de Colombia. La muestra está compuesta de 2231 personas con enfermedad crónica y cuidadores familiares. Se emplearon los instrumentos <em>Encuesta de caracterización para el cuidado de una diada persona con enfermedad crónica-cuidador familiar y Competencia para cuidar en el hogar GCPC-UN-CPC, versión paciente y cuidador familiar. </em></p><p class="p1">Del mismo modo, se utilizó estadística descriptiva para analizar la competencia para cuidar en el hogar y sus dimensiones a partir de los niveles de estratificación alto, medio y bajo; para la comparación se utilizaron pruebas no paramétricas en muestras independientes. </p><p class="p1"><strong>Resultados.</strong> Tanto las personas con enfermedad crónica como los cuidadores familiares tienen niveles heterogéneos de competencia para cuidar en el hogar en las diferentes regiones del país. En ambos casos los niveles superiores se encuentran en la Región Amazónica y los más bajos en la Pacífica. Estos niveles de competencia para cuidar en el hogar distan de ser adecuados para garantizar la calidad y seguridad que se requiere. </p><p class="p1"><strong>Conclusión.</strong> Los indicadores de cuidado continuo deben tener en cuenta la diada receptor-cuidador familiar y abordar el talento humano, las instituciones y al mismo sistema general de seguridad social en salud para modificar una realidad que, de no ser atendida, resultará agobiante para el país. </p>
Objetivo: determinar la validez y confiabilidad de la Entrevista de Percepción de Carga del Cuidado de Zarit, versión de 22 ítems en español, con cuidadores familiares de personas con enfermedades crónicas en Colombia. Materiales y métodos: estudio metodológico de corte transversal, con 652 cuidadores familiares de personas con enfermedades crónicas, residentes en las cinco regiones de Colombia, con el fin de establecer la validez de constructo, con un análisis factorial y la confiabilidad a través de la consistencia interna determinando el alfa de Cronbach. Resultado: respecto a la validez de constructo, el estudio reporta a partir de la asociación libre de la rotación Varimax la carga total, y en sus dimensiones que incluyen carga interpersonal, impacto del cuidado, y las competencias y expectativas sobre el cuidado. Las cargas factoriales corresponden a los ítems planteados para medir cada una de las dimensiones propuestas. Conclusión: el instrumento Entrevista de Percepción de Carga del Cuidado de Zarit, versión de 22 ítems en español, es una herramienta de fácil aplicación y comprensión en población colombiana de diferente nivel educativo, socioeconómico y cultural; además, mostró ser válido y confiable para evaluar la carga del cuidado en cuidadores familiares de personas con enfermedad crónica.
Background Zika virus (ZIKV) infection during pregnancy has severe consequences on the new-born. The World Health Organization declared the Zika outbreak to be a Public Health Emergency of International Concern (PHEIC) in 2016. Health facilities in the regions most affected by Zika lacked the capacity to respond to the increased demand for contraception. The objectives were to explore healthcare users’ perceptions regarding contraception, Zika prevention during pregnancy and post-abortion care (PAC) services in the context of a Zika outbreak in Tegucigalpa, Honduras, and to follow these services over time. Methods This study was part of a broader implementation research study. We used qualitative research consistent with grounded theory approach. Semi-structured interviews and focus groups were performed with women and their partners who used contraceptive services or received PAC services. Data were collected in two stages from December 2017 to July 2018. Themes explored included contraception, Zika and PAC services. Results Participants had positive attitude towards the use of contraceptive methods and demanded more information on safety, efficacy and on side effects. Health care services were inconsistent in the provision of information on Zika and contraception services. ZIKV vector transmission was known but fewer participants were aware of risk of sexual transmission of Zika. Barriers to access healthcare services included contraceptive and PAC services included distance to healthcare facilities, disorganized admission process, long waiting times and out-of-pocket expenditure to purchase medicines. Furthermore, poor quality, mistreatment and abuse of women seeking PAC was prevalent. Some positive changes were noted over time, such as improvements in infrastructure including improved privacy and cleanliness, removal of fees, requisite to bring clean water to hospital. Conclusions Our results highlight the challenges and areas for improvement in policy and practice related to contraceptive services and PAC in the context of ZIKV infection. Public policies to prevent epidemics should focus more on providing proper sanitation; removing barriers to access and use of effective contraception as human rights priority. Zika epidemic has highlighted weaknesses in health systems that obstruct access to and use of sexual and reproductive health services. The study results call for increased efforts to improve access, especially for women of low socio-economic status and intervene at different levels to eradicate discrimination and improve equity in the provision of health care. Qualitative methods can capture the community perspectives and can provide useful information to develop interventions to improve services.
RESUMENEste artículo es el resultado de la revisión temática de fuentes primarias acerca de la metasíntesis, considerada como una metodología investigativa novedosa para las ciencias de la salud, que permite valorar la evidencia científica de la investigación cualitativa y fortalecer el cuerpo de conocimientos de una disciplina al construir, describir y/o explicar fenómenos. Se presentan las definiciones de metasíntesis propuestas por varios autores que coinciden en afirmar que el propósito de ésta es aumentar la interpretación de los resultados de estudios cualitativos generando impacto en la asistencia, la investigación y dando respuesta a los problemas de la práctica. Finalmente, se plantean algunos criterios a tener en cuenta al aplicar la metodología.Palabras Clave: Enfermería, investigación cualitativa, literatura de revisión (fuente: DeCS, BIREME). ABSTRACTThis article resulted from a bibliographical review concerning meta-synthesis, considered to be a novel methodology in health science research. Meta-synthesis' importance lies in evaluating high complex scientific evidence from qualitative research and strengthening a discipline's body of knowledge when constructing, describing or explaining related theories. Several authors' definitions of metasynthesis are presented. They coincide in stating that its purpose is to enhance interpreting an outcome by integrating the findings from qualitative research and thus have an impact on support, research and proposals in resolving problems regarding nursing practice. Some criteria to be born in mind when applying this methodology are also mentioned.
RESUMENObjetivo: Determinar la validez de constructo y confiabilidad del instrumento calidad de vida versión familiar en español de Betty Ferrell y cols.Método: Estudio metodológico, descriptivo, de corte transversal, que incluyó caracterización sociodemográfica de 250 cuidadores familiares de personas con enfermedad crónica captados en la ciudad de Bogotá; pruebas de validez de constructo por medio de la técnica de Análisis Factorial Exploratorio y de confiabilidad determinando estabilidad, a través del cálculo de la correlación entre los totales del test y el re-test, y consistencia interna obteniendo el coeficiente alfa de Cronbach entre todos los ítems del instrumento y de manera específica para cada dimensión.Resultados: La validez de constructo del instrumento calidad de vida versión familiar en español de Betty Ferrell y cols. evidencia la existencia de cuatro factores: dimensión física, psicológica, social y espiritual. El número de factores resultante de la solución del análisis factorial exploratorio se aproximó al número de dimensiones planteados por los autores, las cargas factoriales de casi el 50% de los ítems corresponden exactamente con los ítems diseñados y planteados para medir cada dimensión. Se observa discrepancia en las dimensiones psicológica y social. Respecto a las pruebas de confiabilidad, en la evaluación de la estabilidad se encontró una correlación test-retest de 0.86, y en la de la consistencia Interna un alfa de Cronbach de 0.861 en el test y de 0.866 de re-test.Conclusiones: La escala calidad de vida versión familiar en español de Betty Ferrell y cols. reporta una validez de constructo aceptable y una alta confiabilidad para ser utilizada en el contexto colombiano. Se recomienda consolidar la validez de constructo utilizando técnicas de análisis factorial confirmatorio que permitan estimar el aporte de un ítem a una o varias dimensiones simultáneamente así como la estimación de posibles correlaciones existentes entre las dimensiones.
