Il fumo di tabacco esercita la propria azione nociva sull'organismo attraverso migliaia di sostanze ed elementi. Tra essi, i metalli sono significativamente presenti e per alcuni gli effetti proinfiammatorio e cancerogeno, diretto e indiretto, sono ben noti. Il cadmio è un fondamentale agente tossico presente nel tabacco che, a causa dell'alterazione della clearance muco-ciliare tipica del fumatore cronico, nonché della sua lunga emivita, può esercitare per molto tempo la sua azione, giocando un ruolo importante nella patogenesi di molte patologie fumo-correlate. L'identificazione di questo agente dovrebbe indurre l'industria dei prodotti del tabacco a investire seriamente per l'applicazione di strategie di de-cadmiazione che possano ridurne (o azzerarne) la presenza nella pianta di tabacco, o durante la lavorazione della foglia. D'altro canto è stato dimostrato che nei fumatori cronici, un adeguato apporto di zinco e/o l'utilizzo di farmaci che riattivano la clearance mucociliare possono ridurre l'incidenza e/o la gravità di diverse patologie fumo-correlate anche perché contrastano la presenza e l'attività del cadmio nell'organismo.
