Introdução: A estética do sorriso tem diversos parâmetros na literatura, sendo muito discutida e valorizada no contexto da odontologia. Uma das principais insatisfações dos pacientes é a exposição excessiva de gengiva. O sorriso gengival é uma alteração diagnosticada pelo excesso ou desalinho da gengiva aparente durante o movimento dos lábios superiores. O tratamento do sorriso gengival pode ser realizado utilizando softwares para análise facial, exames imaginológicos e um guia cirúrgico duplo para garantir maior previsibilidade na execução do tratamento. Objetivo: O objetivo deste artigo foi descrever o uso do planejamento digital e do guia cirúrgico duplo no aumento de coroa clínica estético para correção do sorriso gengival, enfatizando os parâmetros utilizados para definir o método cirúrgico utilizado. Relato de caso: Paciente relata incomodo no excesso de exposição de gengiva durante o sorriso. Após a realização de exame clínico e complementares, como a tomografia computadorizada de feixe cônico (TCFC-TM), foi realizado o diagnóstico de hipermobilidade do lábio superior e erupção passiva alterada (EPA). O tratamento escolhido para esse caso foi gengivoplastia associada com osteotomia. Para um melhor resultado do tratamento foi realizado um guia cirúrgico. Após o tratamento a paciente foi orientada e medicada. Conclusão: A execução do planejamento clínico proporcionou uma abordagem cirúrgica com o resultado desejado, apresentando uma harmonia do sorriso através do alinhamento gengival.
