Objetivo: Apresentar o relato de caso de uma paciente que evoluiu com um hematoma subcutâneo em face, após bloqueio anestésico durante um procedimento cirúrgico odontológico.Relato de caso: Uma paciente de 22 anos de idade foi admitida no Serviço de Cirurgia Oral e Maxilofacial do Hospital Federal dos Servidores do Estado do Rio de Janeiro para extração cirúrgica dos terceiros molares inclusos superiores e inferiores. Apresentou um aumento de volume associado a um hematoma de grande proporção em hemiface direita imediatamente após a infiltração rotineira da maxila para bloqueio do nervo alveolar superior posterior. A paciente foi acompanhada por duas semanas até a remissão dos sinais clínicos, sem necessidade de nova intervenção cirúrgica.Conclusão: O hematoma pode se apresentar como uma complicação anestésica comum. Nesse sentido, reconhecer o quadro clínico precocemente e seguir um tratamento adequado é importante para o manejo rápido e seguro do paciente.
