Apresentação dos casos: Relatamos o caso de três pacientes com Síndrome de Budd-Chiari (SBC) e revisamos as manifestações clínicas e métodos diagnósticos. Caso 1: 18 anos, feminino, previamente hígida, iniciou com aumento progressivo do volume abdominal, associado a dor e ganho de peso de 8 kg. Ao exame abdome globoso, flácido, indolor, fígado palpável a 2 cm do rebordo costal direito (RCD), traube livre. Caso 2: 27 anos, sexo masculino, previamente hígido evolui com dor e distensão abdominal associado a melena. No exame físico abdome globoso, ascítico, indolor, fígado não palpável e baço a 3 cm do rebordo costal esquerdo Laboratório com deficiencia de Proteina C e de Fator V da coagulação. Caso 3: 51 anos, masculino com relato de dor e aumento do volume abdominal, icterícia, ascite tensa e hematêmese. Ao exame abdome globoso, tenso, difícil palpação, doloroso difusamente, sem sinais de irritação peritoneal, traube livre e fígado a 4 cm do RCD. Hepatite crônica pelo vírus da hepatite B e carcinoma hepatocelular, alfafetoproteina > 2000. Discussão: A SBC agrupa diferentes processos patológicos cujo ponto comum é a obstrução parcial ou total do fluxo venoso hepático. Na maioria das vezes esta obstrução é devido a trombose das veias hepáticas, todavia, pode decorrer da presença de membranas na luz da veia cava inferior causando obstrução do fluxo venoso hepático, compressão da veia hepática por tumorações neoplásicas ou infecciosas ou por sequela de lesões traumáticas. Essa obstrução venosa aumenta a pressão nos sinusoides hepáticos que, se for suficientemente intensa, ocasionará uma necrose periveia centrolobular. Caso persista o processo obstrutivo, pode resultar em fibrose e cirrose hepática. Excepcionalmente essa a necrose afeta uma área muito extensa, podendo resultar em um quadro de insuficiência hepática fulminante. É mais comum em mulheres entre 20 e 30 anos de vida. Em mais de 80% dos casos identifica-se um distúrbio hipercoagulável subjacente, sendo que, em até 50% decorre de algum transtorno mieloproliferativo crônico. As manifestações clinicas mais comuns são ascite, dor abdominal, icterícia, hemorragia digestiva e encefalopatia hepática. A ultra-sonografia abdominal com Doppler é o exame mais comumente utilizado para o seu diagnóstico. Comentários Finais: A SBC é uma doença de etiologia variada, pode levar a hepatopatia crônica e suas complicações necessitando seguimento clinico para melhora na qualidade de vida desses pacientes. Instituição: PONTIFÍCIA UNIVERSIDADE CATÓLICA DE GOIÁS E UNIVERSIDADE FEDERAL DE GOIÁS Introdução: As varizes de fundo gástrico (VFG) surgem como consequência da hipertensão portal. O sangramento decorrente da sua ruptura geralmente apresentam altas taxas de morbi-mortalidade e são de difícil tratamento endoscópico. A escleroterapia endoscópica com Cianoacrilato (EEC) tem demonstrado ser uma opção eficaz e segura para o controle do sangramento por VFG. Objetivo: Avaliar a eficácia da EEC para a profilaxia secundária do sangramento por ruptura de VFG. Método: Foi avaliada, retrospectivamente o prontuário dos pacientes submetidos a EEC no período de janeiro de 2015 a junho de 2017. Coletamos dados referentes ao gênero, idade, os achados endoscópicos e resultado tardio (após 30 dias) da EEC. Resultados: Durante esse período foram realizadas a EEC em 6 pacientes, sendo que em um deles houve a necessidade de realizar duas sessões pois o mesmo apresentou recidiva hemorrágica após 24 horas. Destes, 03 (50%) pacientes eram do sexo feminino e a média de idade do grupo foi de 40 anos. Em relação ao tipo de VFG, 02 (30%) apresentavam varizes gastroesofágicas (GOV) tipo 1, 03 apresentavam GOV tipo 2 (50%) e 01 (20%) apresentava GOV tipo 1 e 2. 5 (83%) pacientes apresentaram resposta adequada após uma sessão de EEC, nenhum paciente apresentou complicação precoce ou tardia relacionada ao procedimento. Conclusão: A EEC demonstrou ser um método seguro e eficaz para a profilaxia secundária do sangramento por ruptura de VFG.
