Nitrous oxide (N2O), a long-standing anesthetic, is known for its recreational use, and its consumption is on the rise. Several case studies have reported neurological and psychiatric complications of N2O use. To date, however, there has not been a study using standardized diagnostic procedures to assess psychiatric comorbidities in a patient consuming N2O. Here, we report about a 35-year-old male with magnetic resonance imaging confirmed subacute myelopathy induced by N2O consumption, who suffered from comorbid cannabinoid and nicotine dependence as well as abuse of amphetamines, cocaine, lysergic acid diethylamide, and ketamine. Additionally, there was evidence of a preceding transient psychotic and depressive episode induced by synthetic cannabinoid abuse. In summary, this case raises awareness of an important mechanism of neural toxicity, with which physicians working in the field of substance-related disorders should be familiar. In fact, excluding N2O toxicity in patients with recognized substance-related disorders and new neurological deficits is compulsory, as untreated for months the damage to the nervous system is at risk of becoming irreversible.

Heart failure (HF) is an increasing cause of cardiovascular morbidity and mortality. However, the lay public awareness about HF is shown to be inferior to the knowledge of myocardial infarction or stroke. The vast majority of HF patients lack basic comprehension of HF and self-care, which translates into suboptimal treatment outcomes. Therefore this study aimed to establish the present level of knowledge about HF of the lay public and to evaluate the effects of a single public lecture for the consideration of implementing further steps to raise public awareness. Design and methods: We performed a cross sectional survey study using an anonymous questionnaire designed by the German Competence Network Heart Failure (CNHF). Respondents were selected randomly and fulfilled the questionnaire free-willingly prior to and after a standardized lecture on HF. The CNHF questionnaire consisted of two sets of questions focusing on individual characteristics of the respondent and HF-related knowledge. The gathered data were verified by a standardized procedure in Wurzburg, Germany. Results: Among 1025 respondents 60% were females: 15% of the participants had occupational experience in the medical field. The majority (79%) of the study population had heard about HF previously. Typical complaints and symptoms of HF were correctly identified as ‘shortness of breath’ by 69%, ‘swelling of feet’ by 56%, ‘fatigue’ by 56%, and ‘weakness’ by 54% survey participants. The lecture resulted in 22% (from 26% to 48%) increase of correct identification of the combination all three HF symptoms (shortness of breath, swelling of feet and tiredness). The lecture significantly increased the proportion of correct answers for most of the questions. Particularly notable gains were observed in respondents without prior medical experience. Conclusion: The study population’s initial awareness of HF was insufficient. Standardized lecture improved the short-term knowledge about HF symptoms and nature of the disease. Regular educational activities increasing the HF awareness of general population and HF patients can increase the overall HF treatment success.

Širdies, krūtinės ir kraujagyslių chirurgijos klinika Raktažodžiai 91,0, 89,9, 90,1 ir 75,0 proc. atvejų, o veninėse -79,9, 68,6, 57,4 ir 51,0 proc., atitinkamai. Išvados ĮvadasLigonių, kuriems atlikta miokardo revaskulizacijos operacija, tiek gyvenimo trukmė, tiek jo kokybė tiesiogiai priklauso nuo operacijos metu suformuotų vainikinių jungčių funkcionavimo. JAV kasmet atliekama daugiau kaip 350 tūkst. miokardo revaskulizacijos operacijų (1), Lietuvoje 2003Lietuvoje -2004Lietuvoje m. atlikta iki 1800Lietuvoje -2000. Dauguma tyrimų, kurių metu tirtas jungčių pratekamumas, baigties tašku autoriai nurodo laiko trukmę nuo buvusios operacijos iki pooperacinės koronarografijos. Be to, dažniausiai įrodo-mas arterinių jungčių pranašumas lyginant su veninėmis. Arterinė jungtis, panaudojant kairiąją vidinę krūtinės arteriją (KVAK), dažniausiai suformuojama su priekine tarpskilveline šaka (PTŠ) ir (ar) su jos pirmąja diagonaline šaka, o veninės jungtys -su likusiomis vainikinėmis arterijomis. Pateikiamas 40-50 proc. veninių jungčių praeinamumas po 10 metų, o arterinių -90-95 proc. (4-7). Tačiau pastaraisiais metais pasirodo vis daugiau straipsnių, kuriuose pabrėžiama mažo laipsnio susiaurėjimo natyvinėse arterijose reikšmė arterinių jungčių užsidarymui (8, 9).Šio tyrimo tikslas -ne tik palyginti veninių ir arterinių jungčių išgyvenimą, bet ir jų funkcionavimą skirtingose miokardą maitinančių vainikinių arterijų zonose. Jungčių rūšys (arterinė ar veninė) bei jų suformavimo vieta pateikiama 2 lentelėje. Kairioji vidinė krūtinės arterija buvo panaudota priekinės tarpskilvelinės šakos revaskulizacijai, kaip viena ar sekvencinė jungtis su pirmąja diagonaline šaka. Kairioji vidinė krūtinės arterija panaudota 520 (75,9 proc.) ligonių, iš kurių penkiems suformuota po dvi jungtis, dešinioji -12 ligonių, o prieskrandžio -vienam ligoniui. Likusios 1644 jungtys suformuotos panaudojant kojos didžiąją poodžio veną. Jungtis su priekine tarpskilveline šaka atlikta 106 (6,4 proc.) ligoniams, su pirmąja diagonaline šaka -364 (22,1 proc.), su gaubiamąja šaka -629 (38,3 proc.), o su dešiniąja vainikine arterija -545 (33,1 proc.) ligoniams. Rezultatai ir jų aptarimasAnksčiau mūsų publikuotame straipsnyje buvo teigiama, kad koronarografijos, atliktos daugiau kaip po 120 mėn., metu arterinėse jungtyse geras pratekamumas rastas net 76,5 proc. ligonių, o veninėse -tik 34,6 proc. (3). Šio tyrimo metu mes pastebėjome geresnį tiek arterinių, tiek veninių jungčių išgyvenimą (1 pav.).
