As porfirias são moléstias decorrentes de alterações do metabolismo das porfirinas, que se traduzem clinicamente por alterações cutâneas, digestivas, neurológicas e psíquicas. A porfirina 6 é um composto tetrapirrólico que entra no metabolismo de dois grandes grupos de substâncias fundamentais para a vida: um deles é o da protoporfirina férrica que entra na composição da hemoglobina, mioglobina e de vários fermentos, como a catalase, peroxidase e citocromos; outro grupo importante é o dos derivados magnesianos da porfirina, dos quais a clorofila é o principal.Existem vários compostos que derivam do metabolismo da porfirina. A coproporfirina normalmente é eliminada na urina e nas fezes de indivíduos sadios; somente na urina são eliminados cerca de 300 mcg de coproporfirina diariamente. As uroporfirinas não são encontradas nos excretas de indiví-duos sãos, a não ser em quantidades mínimas. Existem vários tipos, tanto de uro como de coproporfirinas, que se diferenciam apenas espectrofotomè-tricamente.As porfirias são usualmente classificadas em duas categorias: congênita e adquirida. A forma congênita ou eritrdpoiética desenvolve-se durante a vida fetal, sendo reconhecida logo na primeira infância; na urina destes pacientes há grande quantidade de coproporfirina tipo I e de uroporfirina tipo I.As porfirias adquiridas ou hepáticas compreendem duas formas clínicas principais: a aguda intermitente e as coproporfirinúrias secundárias. Estas últimas são observadas no decurso de certas afecções agudas e caracterizam-se pelo aumento da excreção das coproporfirinas tipo I e III.
