1. The humoral immunosystem is impaired in those cases of catabolic metabolism in which quantitative and qualitative insufficient alimentation is expected after operation. 2. We showed that after cardiac surgery with extracorporal circulation as well as after other operations a reasonably balanced parenteral nutrition composed of carbohydrates and amino acids could considerably inhibit the changes in the humoral immunity, which is a valuable contribution to the prevention of infection. 3. In our experiments on transformation of lymphocytes after mitogenic stimulation, the depressive effects of operation on the cellular immunity were found to be etiologically more complex than was previously assumed in publications.
Die Beobachtung, daft Tuberkelb~kterien unter 1/~ngerer Isonikotins/iurehydrazid(INtt)-Behandlung einen Virulenzverlust bei der experimentellen !V[eerschweincheninfektion erfahren, ist durch die fibereinstimmenden :Befunde zahlreieher Untersucher 1, 4-:,iv,is, as, 39, 50, 6s-:2,75,TS,93,,~4,97 gesichert. Sie verdient insofern das besondere Interesse der Klinik, als die tnberkulos~atisehe Behandlung trotz ihrer unbestrittenen AnfangserfoIge mit 1/~ngerer Dauer, wohl infolge einer zunehmenden bakteriellen Chemoresistenz der Tuberkelbakterien, an Wirksamkeit verlier~.Vor allem bei sehweren chronisehen Tuberkuloseformen befindet sigh daher die Chemotherapie oft in einer schwierigen, wenn nicht aussichtslosen L~ge. In dieser Situation k51mte die therapeutisehe Nutzbarmachung des Prinzips einer bakteriellen Virulenzschiidigung unter l-NI-I-Einwirkung einen wesentliehen Fortschritt ffir die Behandlung der mensehlichen Tuberkulose bedeuten. Die Forderung naeh langfristiger :[N-/t-Therapie, wie sie yon manehen Autoren 1~ 37,sa,se,sv befiirwortet wird, verffigt in den genannten tierexperimentellen Virulenzbefunden fiber ein beachtliches Argument. Unter diesen Aspekten wiirde zugleieh die Chemoresistenz gegen LNH ihre bisherige Bedeutung weitgehend einbfi/~en; angesichts ihrer engen ]3eziehungen zur Virulenzminderungl, 4-7,15, 21, as, s9, so, 52,ss,7,,TS, SO, Sl,95,ss miil~te sie geradezu als eine unvermeidliche Begleiterseheinung der Virulenzsch//digung in Kauf genommen werden33, l~ Inwieweit diese Vorstellung den ~atss Verh/tltnissen tier menschliehen Tuberkuloseinfek~ion gerecht werden, h/~ngt davon ab, ob die Pathogeni~/~ts-minderung der Tuberkelbakterien im Meersehweinehenversueh in ahnlicher Form aueh ffir ihr Verhalten gegenfiber anderen Wirtsorganismen, speziell dem menschlichen KSrper, giiltig ist. Bei dem wei~gehend empirisehen Charakter unseres Wissens um die Virulenz kann dies nicht ohne weiteres vorausgesetzt werden.
