RESUMO OBJETIVOS.Acidente vascular encefálico isquêmico (AVEi) representa a terceira maior causa de morte e gastos anuais. A literatura demonstra maior ocorrência no período da manhã e em épocas mais frias, podendo ter uma maior prevenção nestes momentos. Portanto, os objetivos foram identificar os fatores de risco, avaliar a influência das variações circadianas e de temperatura na prevalência do AVEi. MÉTODOS. Foram estudados 100 pacientes com o diagnóstico de AVEi. O horário de instalação foi analisado em quatro períodos, sendo obtido do paciente ou dos familiares. A temperatura climática foi obtida do INMET. RESULTADOS. O AVEi foi classificado de acordo com os subtipos estabelecidos pelo projeto TOAST. Dos pacientes, 66% tinham hipertensão arterial como principal fator de risco. Houve um predomínio de casos no período das 6h00 às 11h59 (p< 0.0001) e após variação negativa da temperatura (p < 0,0001). CONCLUSÃO. Encontrou-se uma forte relação entre a prevalência de eventos encefálicos isquêmicos e variação negativa de temperatura e que o horário de maior frequência foi entre 6h00 e 11h59. Não foi possível avaliar as variações de temperatura e circadianas para cada subtipo de AVEi, pois o número de casos estudados não permitiu avaliação estatística significante.UNITERMOS: Acidente cerebral vascular. OBJETIVO: Avaliar retrospectivamente os resultados de uma série de pacientes submetidos à corpectomia torácica e/ou lombar por via exclusivamente posterior associado à colocação de cage e à artrodese instrumentada segmentar e descrever a técnica cirúrgica. MÉTODOS: Foram avaliados retrospectivamente seis pacientes portadores de colapso vertebral, instabilidade biomecânica ou lesão neurológica por diferentes etiologias. Estes pacientes apresentavam indicação de descompressão neural e receberam indicação para a realização de corpectomia e reconstrução circunferencial com cage sendo realizado por via exclusivamente posterior. RESULTADOS: Quatro pacientes eram do sexo masculino e dois do sexo feminino. A idade média foi de 58 anos (22 a 82 anos) com tempo médio de acompanhamento de 10,5 meses (2 a 24 meses). Em três casos a ressecção foi de um corpo vertebral e em três casos de dois corpos vertebrais. Todos os pacientes apresentaram melhora do seu estado neurológico e melhora da dor lombar ou radicular. As indicações ao procedimento foram três casos por espondilodiscite, um por fratura osteoporótica, um tumor metastático e um tumor primário. Três pacientes apresentaram complicações necessitando de revisão cirúrgica evoluindo com melhora dos sintomas. As complicações foram fístula liquórica, radiculopatia lombar, infecção de ferida operatória, meningite e falha da instrumentação. CONCLUSÃO: Os pacientes submetidos à corpectomia por via exclusivamente posterior apresentaram resultados favoráveis com melhora do déficit neurológico ou dor em todos os casos. Esta técnica mostrou-se eficiente na reconstrução circunferencial da coluna evitando as complicações da abordagem tradicional por dupla via.
Objectives: Considering the epidemiological and functional importance of spinal pathologies and the large number of surgeries performed today, the study aimed to evaluate the impact of spinal arthrodesis and discectomy surgeries on patients’ quality of life. Methods: This is a retrospective, descriptive, and longitudinal study developed in the neurology and neurosurgery department of a strategic tertiary hospital in an inland town in the state of São Paulo. Data were collected through a telephone interview, using the Oswestry questionnaire (ODI) to assess lumbar pain in patients submitted to surgery in 2014 and 2015. Results: There was an improvement in the ODI results in all the periods analyzed. When the evaluations were subdivided by surgical type, there was an absolute improvement in the median ODI results in all procedures, however, only the cervical spine arthrodesis procedure was not statistically significant, probably due to the low number of procedures analyzed (n = 12). Conclusion: It can be concluded that the current surgical technique can contribute to the improvement of patients’ quality of life. Level of Evidence III; Comparative retrospective study.
Resumo Objetivos?Avaliar a correla??o entre a express?o do Ki-67 e do p53 em astrocitomas graus II a IV, as caracter?sticas da RM e seu progn?stico. M?todos e Resultados?Este ? um estudo longitudinal retrospectivo do tipo coorte hist?rico que avaliou 45 pacientes. O progn?stico foi avaliado mediante revis?o dos prontu?rios. A avalia??o imuno-histoqu?mica foi realizada no Servi?o de Patologia do HB. A avalia??o das imagens de RM foi feita no Servi?o de Radiologia. A sobrevida m?dia dos pacientes foi de 581,4 dias no grau IV; 1.704,8 dias no grau III; e 1.044,8 dias no grau II. Para cada grau, os seguintes percentuais de c?lulas positivas de Ki-67 e p53 tiveram respectivamente: 97,8 e 80% de positividade no grau IV; 72,7 e 73,4% de positividade no grau III; e 70 e 80% de positividade no grau II. Observou-se diferen?a estat?stica na express?o de Ki-67 entre os graus III e IV (P?=?0,014). A RM mostrou especificidade de 64% e sensibilidade de 100% para a presen?a de necrose tumoral. Conclus?o?Conclu?mos que a an?lise da positividade do Ki-67 e do p53 n?o permite sua correla??o com a sobrevida dos pacientes, apesar de ambos terem sua frequ?ncia distribu?da de maneira semelhante. A RM tem especificidade moderada em rela??o ao exame patol?gico.
