Introdução: A tireoidite subaguda é descrita como causa frequente de dor na topografia da tireoide, tendo sua origem vinculada à infecção viral ou pós-viral. Com o advento da pandemia pelo SARS-CoV-2, raros casos dessa afecção da tireoide foram descritos associados a Síndrome Respiratória Aguda pelo coronavírus. Objetivos: Relatar o caso de uma tireoidite subaguda relacionada a infecção por SARS-CoV-2. Descrição do caso: Descrevemos as características clínicas, laboratoriais e radiológicas de uma mulher de 53 anos que manifestou recorrência da febre após cerca de 15 dias da positivação do swab orofaríngeo para SARS-CoV-2 e persistência da odinofagia mesmo depois da resolução dos sintomas iniciais da infecção viral. Discussão: Na investigação diagnóstica, os valores de TSH e de T4 e T3 livre corroboraram com a hipótese de tireotoxicose, além dos anticorpos antitireoidianos, que se revelaram negativos. Ecografia tireoidiana com doppler revelou ecogenicidade reduzida, dimensões assimétricas às custas de aumento do lobo direito, sugerindo áreas focais de tireoidite. No exame físico, a paciente apresentava frequência cardíaca aumentada e tireoide dolorosa à palpação. Dois meses antes, a função tireoidiana e a avaliação ultrassonográfica estavam normais e não condiziam com os presentes achados descritos. Foi então diagnosticada tireoidite subaguda e iniciado tratamento com prednisona, com resolução da odinofagia após tratamento. Conclusão: Relatamos um caso raro de tireoidite subaguda após infecção por SARS-CoV-2 para alertar a comunidade médica sobre as manifestações clínicas adicionais e não comumente relatadas nas infecções associadas ao COVID-19.
