Background The aim of this study was to evaluate whether Interleukin-6 (IL-6) could be a faster indicator of treatment success in adults with severe sepsis and septic shock compared to procalcitonin (PCT) and C-reactive protein (CRP). Methods Data from adult patients with severe sepsis and septic shock managed at the medical intensive care unit (ICU) of the University Hospital Leipzig between September 2009 and January 2012 were analyzed retrospectively. Values for CRP, PCT and IL-6 on admission as well as after 24 and 48–72 h were collected. Antibiotic therapy was defined as clinically successful if the patient survived ICU stay. Results A total of 328 patients with severe sepsis and septic shock with adequate data quality were included. After 48–72 h, the median IL-6 was significantly lower in survivors than in non-survivors (114.2 pg/ml vs. 746.6 pg/ml; p < 0.001), while there was no significant difference for PCT (5.6 vs. 4.9 ng/ml; p = 0.586) and CRP (158.5 mg/l vs. 172.4 mg/l; p = 0.988). Conclusions The results of this study suggest that IL-6 is better than PCT and CRP in predicting the treatment success in predominantly non-surgical sepsis in the first 48–72 h.
A 31-year-old man sought medical help for suspected foreign-body aspiration. He had been suffering from recurrent pulmonary infections with intense coughing for six months. Repeated courses of antibiotics had failed to bring about permanent improvement, and computed tomography of the chest showed partial atelectasis of the right lower lobe distal to a foreign body.Bronchoscopy revealed the aspirated object in the right lower lobe bronchus, with massive surrounding mucosal irritation. The object was successfully removed with a Dormia basket and identified as a tooth. The patient reported having undergone a difficult wisdom tooth extraction in which the tooth could not be removed in one piece and a fragment of the extracted tooth was noted to be missing. It had been assumed that the patient had swallowed it.Foreign-body aspiration can cause recurrent pulmonary infection. Even in young patients, aspiration should be considered in the differential diagnosis. Patients with chronic cough (>8 weeks) should be evaluated with a chest x-ray and a pulmonary function test. The same is recommended for patients with community-acquired pneumonia that takes a complicated course.
Verschiedene Erkrankungen und Verletzungen können sowohl im Rettungsdienst als auch in der Notaufnahme die Anlage einer Thoraxdrainage (TD) notwendig machen. Dabei stellt das Thoraxtrauma sicher die häufigste Indikation zur Anlage einer TD dar. Im Rahmen eines Polytraumas muss mit dem Vorliegen eines Hämatopneumo-thorax zwischen 9-50 % gerechnet werden [1]. Bei internistischen Erkrankungen kann es zu einem primär spontanen Pneumothorax (mit oder ohne Begleiterkrankung) oder zu einem sekundären Pneumothorax (z. B. bei Vorliegen eines Lungenemphysems oder Status asthmaticus) kommen. Oft kann, v. a. bei klinisch unauffälligem Befund, präklinisch abgewartet und auf die Anlage einer TD verzichtet werden. In der Klinik hat man den Vorteil, mittels Bildgebung (z. B. Röntgen, CT, Sonografie) eine weitere Sicherung herbeizuführen. Eine absolute und dringliche Indikation zur Anlage einer TD stellt ein Spannungspneumothorax dar [1]. Klinisch können im Rahmen einer Spannungssituation folgende Zeichen auffällig sein: ■ ein auf der betroffenen Seite abgeschwächtes oder aufgehobenes Atemgeräusch, ■ der betroffene Hemithorax kann in Inspirationsstellung verbleiben, ■ mittels Perkussion lässt sich ein hypersonorer Klopfschall nachweisen, ■ unter Beatmung können steigende Beatmungsdrücke nachweisbar sein, ■ der Blutdruck kann sinken und die Herzfrequenz steigen (Kreislaufdepression). Vor Anlage einer TD sollte evaluiert werden, ob gerinnungshemmende Medikamente eingenommen werden, oder aber eine Gerinnungsstörung anamnestisch bekannt ist. Bei einer Spannungspneumothoraxsituation muss auch unter diesen Bedingungen eine rasche Entlastung des Pleuraraumes herbeigeführt werden. Innerklinisch sollte (bei stabilen Vitalfunktionen) vor Anlage eine Röntgenthoraxaufnahme angefertigt und eine Kontrolle der Laborwerte vorgenommen werden, solange kein sicherer Spannungspneumothorax vorliegt. Indikationen und Kontraindikationen für die Anlage einer TD sind in Tabelle 1 zusammenfasst. Vorgehen Es werden prinzipiell 3 Techniken zur Entlastung des Pleuraraumes unterschieden. Einerseits die Anlage der TD nach Monaldi und andererseits nach Bülau. Darüber hinaus kann in einer dringlichen Situation (z. B. akuter Spannungspneumothorax) durch Punktion mit einer Flexüle (sog. Nadeldekompression) in Monaldi-Position (2./3. Interkostalraum medioclaviculär) eine passagere Entlastung vorgenommen werden. Es muss jedoch zeitnah (d. h. bei prähospitaler Nadeldekompression auch
