Case: A total knee arthroplasty (TKA) was performed on a 35-year-old man with congenital fibular deficiency and a 20° varus and 28° antecurvatum tibial deformity of the left lower limb. Conclusion: One-stage TKA and correction of the extraarticular deformity by means of intraarticular bone resections and a standard soft tissue release were performed to restore the limb's mechanical axis. Patients with congenital fibular deficiency present a wide range of limb deformities because of bone deficiencies or treatment sequels, which might require a specific surgical technique and small-sized implants to obtain good results during a TKA.
Case: A modified induced membrane technique for the treatment of a posttraumatic 10-cm infected segmental femoral defect is reported. The first stage involved debridement and insertion of an antibiotic cement spacer and 2 Ender nails. Reconstruction was performed with a 14-cm structural bone allograft, inverted and telescoped into both ends of the remaining host femur, and stabilized with an interlocking nail. Progressive weight-bearing was allowed at 3 weeks with full weight-bearing achieved at 6 months. Conclusion: The use of a telescoped structural allograft and intramedullary nailing as the second stage of the Masquelet induced membrane technique facilitated postoperative rehabilitation, increased surface contact between host and graft bone, and improved integration.
RESUMENIntroducción: El éxito a largo plazo de un reemplazo total de rodilla (RTR) depende de la restitución del eje mecánico del miembro, de la correcta colocación del implante y del balance de partes blandas. Los pacientes con gonartrosis asociada a una deformidad extraarticular, femoral o tibial, requieren de una cuidadosa evaluación y planificación preoperatoria para decidir si la corrección del eje podrá realizarse mediante la resección ósea intraarticular, o si deberá asociarse una osteotomía extraarticular. Materiales y métodos: Se evaluaron retrospectivamente 22 RTR (en 20 pacientes) por gonartrosis asociada a deformidad extraarticular. La etiología de la deformidad extraarticular fue postraumática en 8 casos y pososteotomía en 14 (8 femorales y 6 tibiales). El seguimiento míni-mo fue de 5 años. En todos los casos, se realizó la corrección del eje del miembro mediante un RTR con cortes óseos intraarticulares "inusuales", sin necesidad de una osteotomía. Resultados: En 20 de los 22 casos tratados con esta téc-nica, se obtuvieron buenos resultados. Se empleó el sistema de puntaje de la Knee Society para la evaluación clí-nica y funcional. El promedio de la evaluación clínica fue de 24,3 puntos en el prequirúrgico y 86 al año posoperatorio. El puntaje funcional pasó de un promedio de 34 en el prequirúrgico a 85,3 al año. Conclusiones: No hay un criterio único respecto al tratamiento del paciente con gonartrosis sintomática asociada a una deformidad extraarticular. La corrección de la deformidad extraarticular por medio de cortes óseos intraarticulares "inusuales" es posible si estos no afectan las inserciones ligamentarias femorales. Este método puede aplicarse en desejes femorales de hasta 20°y tibiales de hasta 30°en el plano coronal.PALABRAS CLAVE: Reemplazo total de rodilla. Deformidad extraarticular. Cortes óseos inusuales. Patients with osteoarthritis of the knee associated with femoral or tibial extra-articular deformity, require thorough preoperative planning in order to decide if the axis correction could be made through "unusual" intra-articular bone cuts, or if it should be associated with an osteotomy. Methods: Twenty two TKR were performed in 20 patients with osteoarthritis of the knee associated with extraarticular deformity of post-traumatic etiology in 8 cases and post-osteotomy in 14 (8 femoral and 6 tibial). Un varón de 34 años sufrió, en un accidente de tránsi-to con vuelco, una fractura expuesta del tercio distal del fémur derecho (Gustilo III B) y lesión grave de las partes blandas con pérdida de cobertura cutánea y masa muscular del muslo derecho. Fue atendido inicialmente en otro centro, donde se le realizaron múltiples limpiezas quirúr-gicas por una infección recurrente, con cultivos positivos para Acinetobacter baumanii, y recibió minociclina más ciprofloxacina hasta la negativización de los cultivos (se desconocen las dosis). Finalmente se estabilizó el miembro con un tutor externo monoplanar por la cara externa del muslo y se realizó la cobertura del defecto con injertos de piel.El paciente, derivado a nuestro centro a los 6 meses de evolución, presentaba al ingresar dolor en el foco, una cicatriz fibrosa adherida de 15 cm de largo, con déficit de masa muscular y tejido celular subcutáneo, que involucraba de manera circunferencial el 70% del diámetro del muslo derecho, pérdida de capital óseo supracondíleo del fémur derecho de unos 8 cm y un acortamiento del miembro de 15 cm ( Fig. 1A-C).Se retiró el tutor externo y se realizó una limpieza quirúrgica, en la cual se encontró material necrótico en el foco. El segmento de 10 cm distales del fémur proximal presentaba mal aspecto, por lo que se decidió su resección durante el tratamiento definitivo (Fig. 1D). Con cultivos positivos para Enterococcus faecalis sensible a la ampicilina se comenzó el tratamiento antibiótico intravenoso (amoxicilina 1 g cada 8 horas y trimetoprima-sulfametoxazol (TMS) Forte, 1 comprimido cada 8 horas) hasta el alta hospitalaria. Debió completar el tratamiento antibiótico (amoxicilina 1 g cada 24 horas y TMS Forte 1 comprimido cada 24 horas durante un año).A los 45 días se realizó el tratamiento definitivo del defecto óseo con un aloinjerto estructural intercalar con hueso de nuestro banco de tejidos (congelado, no irradiado), de 15 cm de longitud, asociado a injerto molido en los sitios de contacto injerto-huésped, y un clavo endomedular retrógrado acerrojado ( Fig. 2A-D). El paciente realizó marcha con dos muletas con carga del 10% durante el primer mes posoperatorio, con una flexión-extensión de rodilla de 0º a 5º. La carga se fue aumentando progresivamente hasta llegar al 80% al sexto mes, cuando radiográficamente se observaron claros signos de consolidación e integración en las uniones injerto-huésped. Comenzó con ejercicios de flexión y extensión activa al tercer mes de la operación.Con una movilidad de 0°a 20º, eritrosedimentación de 5 mm (VN de 0 a 16) y PCR de 0,2 mg/dl (VN de 0 a 0,3), el paciente dejó la asistencia para caminar a los 10 meses. Puede verse en los controles radiográficos hasta los 3 años la progresiva incorporación y remodelación del injerto (Figs. 2 y 3). Actualmente tiene una movilidad activa de 0°a 75º y realiza una marcha ilimitada e indolora, sin asistencia.
