Se analizan las muertes maternas ocurridas en 25 instituciones hospitalarias que atienden partos en Colombia, en el período comprendido entre el 1o. de julio de 1979 y el 30 de junio de 1980. En este período se atendieron en estas instituciones, 84.387 nacidos vivos con 195 muertes maternas, lo que da una tasa de mortalidad materna de 23.1 X 10.000. Del total de muertes maternas se reqistraron en el formulario precodificado diseñado para tal fin, 145 casos que constituyen el universo de esta investigación. Los resultados de la investigación muestran: 1. La muerte materna se presenta principalmente durante o como consecuencia del parto. 2. Existe un bajo nivel de escolaridad con un deficiente número de controles prenatales en las pacientes que fallecen.3. La mortalidad más frecuente en nuestro medio es la toxemia y las complicaciones primarias del trabajo más frecuentes son el trabajo de parto prolongado u obstruido y la hemorragia. 4. La gran mayoría de las muertes maternas en Colombia carecen de autopsia.5. Las primeras causas de muerte materna son la sepsis por aborto y la toxemia. 6. La muerte materna se acompaña con mucha frecuencia de la muerte del producto de la concepción.
Dentro de la variedad de monstruos ligados al embarazo gemelar, y conocidos con el nombre de Acardius, se encuentra el denominado Hoioacardius Amorphus, que Schatz define como una masa completamente amorfo, sin ninguna apariencia humana y sin rudimentos de corazón.
Sobre un total de 13.450 casos de parto hospitalario en Colombia, recolectados de marzo a octubre de 1977, el presente trabajo analiza 1.168 de estos casos de embarazadas añosas (se han descartado 10 casos, en la mayoría de las veces por razones de defecto de recolección), de los cuales 884 (6.6%), corresponden a mujeres de 35-39 años y 284 (2.1%) a mujeres de 40 a 48 años. Además, se compara este grupo con 11.040 casos de partos en mujeres de 18 a 34 años, correspondiente al mismo estudio.Comparativamente con el grupo más joven, la enfermedad hipertensiva se dobla, el sangrado particularmente, la hemorragia post-parto se hace mayor, las cesáreas se duplican, la permanencia hospitalaria se alarga. La mortalidad materna fue tres veces mayor y la mortalidad perinatal aun en el vértice anterior casi se triplica, siendo desusualmente alta en las cesáreas y en algunos tipos de presentaciones como la de nalgas. Es decir, dentro de categorías específicas de modalidades de parto y atención igual o variedades de presentación similares, la mortalidad perinatal se hace obstensiblemente mayor en la embarazada por encima de los 35 años.A la luz de los diversos hallazgos y cuadros se ha señalado y cuantificado con cifras colombianas, suficientemente representativas y dicientes, el mérito existente para calificar a la embarazada añosa como un riesgo severo en el campo de la atención materno-infantil.
En el Hospital General de Medellin, se aplicaron 300 DIUs, tipo Lippes C y D, por personal paramédico que había recibido adiestramiento previo y con una adecuada supervisión.La recolección de los datos se realizó en dos tipos de formularios precodificados, uno de admisión y otro de seguimiento, el cual se extendió hasta los 6 meses después de aplicado el DIU. Los datos obtenidos fueron procesados por computador y posteriormente analizados. El análisis de los mismos demostró la bondad del procedimiento de delegar la aplicación del DIU a personal paramédico con adecuado adiestramiento y supervisión. No se encontró modificación con la efectividad, tolerancia, efectos secundarios o complicaciones comparándolo con estudios similares realizados en otros países.La principal queja a la inserción fue el dolor y la principal causa de retiro del DIU a los seguimientos fue sangrado y dolor.La anexitis fue una complicación tardía que hace suponer que a mayor tiempo de uso del DIU, existe mayor riesgo de infección pélvica.
