<p>La desinfección del sistema de conductos radiculares nos garantiza el éxito en el tratamiento endodóntico. La utilización de quelantes como el ácido etilendiaminotetraacético (EDTA) es indispensable para lograrlo, pero la dentina sufre cambios estructurales en su topografía que puede provocar Fracaso endodóntico. Se realizo estudio de tipo Cuasi-experimental. Se utilizaron cuarenta conductos amplios y rectos. Los conductos se instrumentaron mediante la técnica de fuerzas balanceadas corono-apical con instrumentos manuales limas Flex-R primera y segunda serie y se irrigó entre cada instrumento con NaOCl. La irrigación final consistió en 3 ml de EDTA al 17% seguido de 5 ml de NaOCl al 5.25%. Se cortó la raíz longitudinalmente con un disco de diamante y se prepararon las muestras con un baño de oro en plasma para poder ser observados en el Microscopio Electrónico de Barrido. Fue analizado el grado de erosión de los tercios medio y apical del conducto radicular. Los resultados encontrados al utilizar el EDTA al 17% mostraron en el tercio medio 50% erosión severa y 25% erosión moderada y en el tercio apical 30% erosión severa y 27.5% erosión moderada. La alteración de la dentina que el EDTA provoca debe ser considerada durante el proceso de obturación.</p>
Durante la preparación biomecánica del conducto radicular se forma una capa llamada lodo dentinario, se han utilizado varias sustancias químicas para su remoción. En nuestro estudio se utilizaron conductos amplios y rectos de sesenta y cinco dientes extraídos. Se dividieron tres grupos experimentales aleatoriamente de 20 conductos y un grupo control positivo de 5 conductos. El grupo control se irrigó con NaOCl al 5.25% como irrigación final. A los grupos restantes se les irrigó con acondicionador «A» a base de EDTA al 17%; acondicionador «B» EDTA al 17% y cetrimida; acondicionador «C» EDTA al 17% y un surfactante y como irrigación final NaOCl al 5.25% y se observaron en el microscopio electrónico de barrido. Los resultados mostraron que el acondicionador «B» logró eliminar el lodo dentinario casi en su totalidad y en donde se utilizaron los acondicionadores «A» y «C» los resultados mostraron una eliminación parcial del lodo dentinario. Estas diferencias encontradas fueron estadísticamente significativas en el grupo del acondicionador «B», cuando fueron analizadas mediante la prueba de χ2 (P < 0.05). Estos resultados fueron similares en el tercio apical y el tercio medio de los conductos radiculares.
<p>En el tratamiento endodóntico pueden ocurrir percances, estos accidentes se definen como aquellos sucesos infortunados que ocurren al realizar la terapia endodóntica, algunos de ellos por una falta de una atención, y otros, por ser totalmente imprevisibles, entre estos últimos, están la perforación en el piso cameral, así como una deficiente obturación del sistema de conductos, lo que condiciona aún más el fracaso del tratamiento. En las perforaciones el tiempo es un factor crucial. El mejor momento para reparar la perforación de la raíz es inmediatamente después de que esta ocurre para reducir al mínimo el potencial de aparición de la infección en el sitio de la perforación. El uso de materiales de restauración como el Mineral Trióxido Agregado (MTA), proveen un mejor pronóstico en el tratamiento de las perforaciones.</p>
