Introducción. La caries dental es una enfermedad multifactorial, que en estados avanzados puede afectar la pulpa dental. Para llevar a cabo un tratamiento exitoso es necesario realizar un buen diagnóstico, ayudándonos de auxiliares como el estudio radiográfico, el cual resulta fundamental para corroborar la posible afectación pulpar, afectación de los tejidos de soporte y la presencia de anomalías dentarias; tal es el caso de las raíces accesorias o supernumerarias, las cuales pueden estar presentes tanto en dentición temporal como permanente, y si no son diagnosticadas pueden complicar o disminuir el éxito del tratamiento odontológico. Caso clínico. Paciente masculino de 7 años 10 meses de edad, con presencia de caninos birradicados bilaterales, que acude a la Clínica de la Especialidad en Estomatología del Niño y del Adolescente, FES Zaragoza, con dolor de cinco días de evolución en diente 53. El diagnóstico fue una pulpitis irreversible, por lo cual, se realizó una biopulpectomía con una evolución posterior, satisfactoria. Conclusión. La detección radiográfica de un canino birradicado, permite realizar un adecuado diagnóstico y tratamiento.
