Bilateral exudative retinal detachment is rarely seen as an ocular manifestation of acute myelogenous leukaemia. Herein, a case is described where a trans-scleral choroidal biopsy was used to diagnose relapsing acute myelogenous leukaemia when the rest of her systemic work-up was negative.
Lactante de 6 meses de edad, enviada para valoración al servicio de Trasplante de células progenitoras hematopoyéticas del Instituto Nacional de Pediatría. Entre sus antecedentes personales: la madre de 24 años, primer embarazo, evolución normal, parto eutócico, peso al nacimiento 3 kg, talla 48 cm y Apgar 8/9. Desde el nacimiento se identificó dermatosis maculopapular generalizada, con nódulos subcutáneos infiltrativos violáceos en todo el cuerpo. (Figura 1) Se estableció el diagnóstico presuntivo de síndrome de neonato blueberry-muffin; además, se encontró hepatomegalia y esplenomegalia. La biometría hemática reportó hiperleucocitosis (215,900 leucocitos/mm3), a expensas de 75% de blastos, con inmunofenotipo de expresión: CD45+, MPO+, CD33+, CD13+ y CD14+, compatible con leucemia mieloblástica aguda.
La leucemia linfoblástica aguda es el padecimiento oncológico más común en pediatría. Es una entidad genéticamente compleja que persiste como causa importante de mortalidad relacionada al cáncer infantil. Su actual éxito terapéutico es el producto de los avances en el entendimiento de su perfil genómico, dando como resultado la precisa asignación del riesgo en función de las características biológicas y clínicas de los pacientes al diagnóstico, así como la respuesta al tratamiento con la modulación adecuada de la intensidad de la quimioterapia.
