9 Patienten mit der DSM-III-R-Diagnose Panikstörung/Agora-phobie wurden hinsichtlich ihres Umgangs mit Konflikten gegenüber ihren Partnern und dessen Veränderung zwischen den Zeitpunkten vor Ausbruch der Krankheit und in der Krankheit befragt. Die Patienten berichteten über eine nicht-signifikante Abnahme von Konflikthäufigkeit und -heftigkeit, aber auch über eine signifikante Abnahme des aggressiven Konfliktstils. Über eine Tendenz in Richtung Zunahme des unterwürfigen und zurückziehenden Stils nach Ausbruch der Krankheit wurde ebenfalls berichtet. Das Partnerverhalten wurde nach Ausbruch der Angststörung signifikant weniger konstruktiv, allerdings auch weniger vermeidend erlebt. Die eingeschätzte Qualität der Beziehung korrelierte vor Ausbruch der Erkrankung nicht mit einem der Konfliktstile, nach Ausbruch der Angsterkrankung allerdings mit dem einge-schätzten Partnerverhalten, und zwar positiv bezüglich Selbst-sicherheit und negativ bezüglich Verleugnung. Hypothesen über Zusammenhänge zwischen Konfliktstilen im Zusammen-leben mit dem Partner und Entstehung bzw. Aufrechterhal-tung der Panikstörung/Agoraphobie werden aufgestellt.
Die deutsche Ubersetzung des «Styles of Conflict Inventory» (SCI) wurde vorerst an 88 Personen (neurotische Patienten und nichtklinische Kontrollpersonen) validiert. Nach einer Faktoren-analyse zeigte sich eine der Amerikanischen Originalversion entsprechende Faktorenstruktur mit guter Trennung der Konstrukte Selbstsicherheit, Aggression, Rückzug, Unterwerfung und Verleugnung. In ersten Studien zur diskriminanten Validität des Instruments konnte unter anderem mittels Diskrimininanzanalyse gezeigt werden, daβ der SCI in der Lage ist, zwischen klinischen und nichtklinischen Gruppen zu differenzieren. So konnten z.B. mittels der Konfliktstil-Skalen 95 % Frauen richtig zur Gruppe der Gesunden oder zur Gruppe der Patienten mit Panikstörung/ Agoraphobie zugeordnet werden. Die Ergebnisse dieser ersten Validierungsstudie zeigen, daβ es sich bei der deutschen Version des SCI um ein reliables und valides Instrument handelt, das überdies von klinischer Relevanz zu sein scheint.
ZusammenfassungEine stadiengerechte Therapie kolorektaler Malignóme setzt die exakte Kenntnis des Tumorstadiums voraus. Intraoperativ ist dies für die Beurteilung der Operationsradikalität und die Planung weiterführender adjuvanter oder palliativer Behandlungsmodalitäten unerläßlich. In einer kontrollierten prospektiven Studie, an der 20 Patienten teilnahmen, wurde versucht, den Stellenwert nuklearmedizinischer Untersuchungen unter diesem Aspekt zu evaluieren. Mit 99mTc markierte Antikörper gegen zelluläre CEA-Epitope wurden zur intraoperativen Szintimetrie eingesetzt. Die Meßergebnisse wurden mit jenen der etablierten Verfahren verglichen und histopathologisch überprüft. Die intraoperative Szintimetrie erlaubt die exakte Lokalisation metastatischer Absiedelungen. Die Radikalität der Operation kann zuverlässig festgestellt werden. Die Immunszintimetrie ermöglicht somit eine exakte Stadienzuordnung zu einem frühen und therapeutisch relevanten Zeitpunkt.
