RESUMOA hanseníase é uma doença crônica e infectocontagiosa que possui como agente etiológico o Mycobacterium leprae. É endêmica em muitas regiões do mundo, sendo um problema de saúde pública no Brasil e uma patologia de notificação compulsória. Considerando a relevância do tema, esse trabalho visa relatar um caso clínico ressaltando a importância de ter conhecimento acerca dos dos diagnósticos diferenciais de hanseníase. Paciente do sexo masculino, 49 anos, com história de lesões de pele há 09 anos. Tratamento para hanseníase multibacilar, neuropatia periférica e eritema nodoso. Em biópsia foi revelado uma dermatite perivascular discreta e superficial podendo ser compatível com eritema polimorfo bolhoso ou síndrome de Sweet. Solicitada nova baciloscopia biópsia que confirmou o diagnostico de hanseníase, descartando a Síndrome de Sweet. As reações hansênicas são fenômenos de aumento da atividade da hanseníase, com piora clínica do quadro inicial, podendo ocorrer em qualquer fase da doença, antes, durante ou após o tratamento. Diante do relato de caso apresentado, evidencia-se a importância do reconhecimento das reações hansenianas e a relevância da investigação dos possíveis diagnósticos diferenciais, visando evitar condutas errôneas e equivocadas.
