Conclusion: The complexity of the procedure and type of stent may play a role in post-CAS hypotension. Commentary: Prolonged hypotension associated with CAS is due to stimulation of the carotid sinus baroreceptors, which are activated at the time of balloon dilation and by the radial forces exerted on the artery wall by the stent. The result is activation of a medullary-mediated parasympathetic response, producing hypotension, and a sympathetic response, producing bradycardia, although both are usually transient.
