ResumoA hipertensão pulmonar é uma síndrome clínica grave, multifatorial e multidisciplinar, com grande prejuízo à qualidade de vida dos doentes e com alta morbimortalidade. O ecocardiograma é o principal exame utilizado para a triagem da hipertensão pulmonar por seu fácil acesso e por ser não invasivo. Este trabalho tem como objetivo analisar a aplicação da ecocardiografia na avaliação morfológica e funcional do coração dos doentes com hipertensão pulmonar. IntroduçãoDefine-se a hipertensão pulmonar (HP) como o aumento da pressão arterial pulmonar média (PMAP) ≥ 25 mmHg em repouso, aferido por cateterismo cardíaco direito (CCD). A PMAP normal foi estimada em 14 ± 3 mmHg, sendo o valor máximo normal 20 mmHg 1 .A HP caracteriza-se por apresentar alterações hemodinâmicas e fisiopatológicas, tornando-se uma síndrome complexa, de difícil diagnóstico e tratamento e, muitas vezes, prognóstico desfavorável [2][3][4][5] . Doentes com PMAP ≥ 25 mmHg possuem diagnóstico de HP e, após essa determinação, deve-se definir se a HP é pré-capilar ou pós-capilar.Se a pressão capilar pulmonar em cunha (PCPC) for ≤ 15 mmHg, a HP é dita pré-capilar, compreendendo a hipertensão arterial pulmonar na ausência de outras causas (grupo 1), a HP de origem pulmonar (grupo 3), o tromboembolismo pulmonar crônico (grupo 4) e a HP de causa pouco clara ou devida a fatores multifatoriais (grupo 5).Se a PCPC for >15 mmHg a HP é do tipo pós-capilar, originada por doenças do coração esquerdo (grupo 2), sendo preciso determinar o gradiente de pressão transpulmonar (GPT), que pode ser obtido pela fórmula 6 : GPT = PMAP -PCPC médiaQuando essa diferença for ≤12 mmHg, o aumento da pressão arterial pulmonar (PAP) é dito passivo, ou seja, causado exclusivamente pelo acometimento cardíaco. Se o GPT for >12 mmHg, o aumento da PAP é desproporcional ao aumento da pressão no Ventrículo Esquerdo (VE) e, portanto, existe remodelamento vascular pulmonar ou outra causa associada para o aumento da PAP.A definição da HP em exercício como PMAP > 30 mmHg, aferida por CCD, não foi validada pelos dados publicados Todos esses fatores influenciam significativamente no desenvolvimento da HP e para a evolução para um quadro extremo denominado Síndrome de Eisenmenger, em que a resistência vascular pulmonar se torna maior que a sistêmica e o fluxo pelo defeito se inverte de maneira irreversível. Esse quadro clínico apresenta alta morbidade devido à hipoxemia e às diversas alterações hematológicas secundárias 4,5 . Diagnóstico Doentes com queixa de dispneia aos esforços, dor precordial, tontura e/ou síncope e sinais de insuficiência cardíaca direita sem causa evidente devem ser avaliados para pesquisa de HP 8 .
