La cetoacidosis diabética (CAD) no es una presentación rara en los hospitales, a pesar de ser una condición totalmente prevenible. La CAD es causada por una disminución de la insulina circulante efectiva asociada con elevaciones de las hormonas contrarreguladoras y se produce principalmente en niños. Esta complicación potencialmente mortal de la diabetes mellitus tipo 1 (T1DM) con frecuencia se maneja mal, lo que genera morbilidad y una mayor duración de la estadía. En tal sentido, se requiere una cuidadosa sustitución de déficit de líquidos y electrolitos, administración intravenosa de insulina y un estrecho seguimiento de parámetros clínicos y bioquímicos dirigidos a la detección oportuna de complicaciones, incluyendo hipopotasemia, la hipoglucemia y edema cerebral. Los factores que aceleran los escenarios de cetoacidosis diabética deben identificarse lo antes posible para evitar estas complicaciones. Sin embargo, las tasas de mortalidad han caído significativamente en los últimos 20 años a <1% esto se debe a los avances en la tecnología de pruebas cercanas al paciente mejorando la atención, al facilitar un diagnóstico rápido y un monitoreo más cercano de la respuesta al tratamiento.
Una de las enfermedades más importantes y complejas de las vías biliares es la estenosis biliar. Es un estrechamiento fijo de un segmento focal de la vía biliar que provoca una dilatación de la vía biliar proximal y parámetros clínicos de ictericia obstructiva. Existe un gran espectro de enfermedades hepatobiliares y pancreáticas, tanto benignas como malignas, pueden provocar el desarrollo de estenosis biliares. La mayoría de estas lesiones “se encuentran asociadas con lesiones iatrogénicas postoperatorias, comúnmente seguidas de colecistectomía (aproximadamente 80%) o posterior a una anastomosis después de una resección hepática o trasplante hepático. La incidencia de las lesiones biliares por colecistectomía laparoscópica oscila entre 0.2 y 1.7%. Las causas de la estenosis biliar son múltiples, en tal sentido, su diferenciación es vital ya que, de acuerdo a esta causa, su tratamiento y pronóstico varían. El objetivo general de la presente investigación consiste en plasmar las principales causas de la estenosis biliar, así como de sus consecuencias. El modelo de investigación es una revisión de tipo documental bibliográfico. Entre sus principales causas se encuentran congénitas, infecciosas, autoinmunes, inflamatorias, neoplásicas, isquémicas y iatrogénicas. Con relación a sus complicaciones, son tan variadas como sus causas. No obstante, se puede mencionar como consecuencia de la estenosis de las vías biliares la cicatrización hepática (cirrosis), hipertensión portal, esplenomegalia, cálculos biliares y piedras en las vías biliares, várices, cáncer de hígado, osteoporosis, carencia de vitaminas, encefalopatía hepática y un mayor riesgo de padecer otra serie de enfermedades. Es fundamental para el profesional de la salud conocer y mantenerse actualizado con los nuevos hallazgos científicos relacionados con esta etiología y mejorar la prognosis de los pacientes con estenosis biliar.
