Introducción: La cirrosis hepática constituye uno de los principales problemas de salud en el mundo, en Ecuador ocupa el séptimo lugar en causas de mortalidad, según datos del INEC. Las complicaciones de esta patología son el principal motivo de internación en las diversas unidades hospitalarias con tasas elevadas de morbilidad y mortalidad. Materiales y métodos: Estudio de tipo observacional, descriptivo, prospectivo y de corte transversal. Se incluyeron los pacientes con diagnóstico confirmado de cirrosis hepática, edad igual o mayor a 20 años, historia clínica en la que se constató el estudio de factores etiológicos y cualquiera de las complicaciones de esta patología, desde enero hasta diciembre de 2019. Resultados: Se incluyeron 185 pacientes con diagnóstico de cirrosis hepática, el sexo femenino predominó con el 56.2%, con una razón hombre:mujer de 1.0:1.3; el grupo etario más frecuente correspondió a aquel entre 60 y 79 años (49.2%). En la escala de Child-Pugh B se encontró el 49.2%; la etiología más frecuente fue la cirrosis por NASH con un 45.4%, seguido por la cirrosis no determinada con el 31.4%, y la cirrosis por alcohol con un 16.8%. Las complicaciones más frecuentes fueron el sangrado por várices esofágicas con el 79.5%, ascitis con un 66.5% y encefalopatía con un 35.1%. Conclusiones: Los resultados de este estudio sugieren que se debe implementar programa de atención y tratamiento del síndrome metabólico, dado que la principal etiología de la cirrosis fue por NASH, así como también, desarrollar medidas preventivas para las complicaciones, como son las várices esofágicas.
El papiloma escamoso del esófago es relativamente raro, tiene una prevalencia de 0.01 a 0.45%. Generalmente es asintomático, de hallazgo incidental durante procedimientos endoscópicos. Sepresenta más en hombres que mujeres, en una relación de 3:1, y su vía de transmisión es de manera directa. La infección oral por virus de papiloma humano se ha asociado a un subgrupo de carcinomas escamosos de la oro-faringe en pacientes con comportamientos sexuales de riesgo elevado. El diagnóstico temprano y oportuno es vital para el pronóstico. Se describen dos casos clínicos de pacientes femeninas, de edad entre 60 a 70 años, con antecedentes de cirrosishepática, que acuden a la consulta de gastroenterología para realización de screening de várices esofágicas, durante el cual se evidenció, como hallazgos endoscópicos y corroborados con estudio anatomopatológico, la presencia de una lesión exofítica, con tallos fibrovasculares, correspondiente a la presentación clásica del virus del papiloma humano.
Cholangiocarcinoma is a rare and aggressive adenocarcinoma that arises from epithelial cells of the biliary tract, the intrahepatic has a very low incidence worldwide < 10% in relation to extrahepatic patients. We present this case of an elderly female patient with no history of importance and risk factors that help us discern her etiology, imaging studies, therapeutic that were provided by the endoscopic retrograde cholangiopancreatography with the placement of 10 x 80 mm partially covered metallic stent for biliary drainage as a palliative measure.
Juvenile polyposis syndrome is an autosomal-dominant condition disease characterized by multiple juvenile polyps in the gastrointestinal tract. These polyps may be present in the entire digestive tract; generally in the colon. We present the case of a 16-year-old adolescent with a history of polyps, who had had bowel resections for acute abdomen on two occasions (at 5 and 11 years of age). Endoscopic procedures were carried out, finding polyps and performing a polypectomy. After 24 hours, the patient presented proctorrhagia associated with acute abdominal pain by an obstruction, for which he was surgically intervened with an exploratory laparotomy where an intussusception was found.
