Background: Infective Endocarditis (IE) is an infection of the endocardia surface of the heart, including the valves. Infective endocarditis (IE) is associated with a broad array of complications and successful treatment of IE relies on microbial eradication by antimicrobial drugs. Surgery contributes by removing infected material and draining abscesses. Nurse's performance plays a role in decreasing morbidity and mortality caused by Infected Endocarditis.
Intra-hospital transportation of critically ill patients within the hospital is potentially dangerous and creates a challenging and highly stressful work, even for experienced nurses because of the severity of the illnesses and the need for continuous monitoring during transportation. The aim of the study was to evaluate the effect of educational program of intrahospital safety transportation of critically ill patients on nurses' performance and patients' clinical outcomes. Setting: This study was conducted at Surgical Intensive Care Units in Emergency Hospital of Tanta Main Hospitals, Egypt. Method: A quasi-experimental design was utilized in this study. Sample of the study consisted of all nurses (70) who are working in the ICU and convenience sampling of 60 critically ill patients, which divided equal into 30 patients of both study and control groups. Tools: Three tools was used to collect data, Tool (1) Socio demographic date of studied nurses, Tool (II) Structured Interview Schedule and Tool (III) patient' clinical outcome. Results: It was observed that vast majority (96%) of the studied nurses reported low level of total knowledge pre implementation of educational program compared with the majority (84%) them reported high level of knowledge post implementation of educational program. Additionally, the majority of the studied nurses (88.6%) had unsatisfactory practice level compared with vast majority (92%) of them had satisfactory level of practice post educational program and there were high statistical improvement on critically ill patients' clinical outcomes during intra-hospital transportation. Conclusion: A well designed safe intra-hospital transportation education program could positively affect the nurses' knowledge and practice and decline the patients' adverse events and complication during transportation Recommendation: It was recommended that encouraging nurses' in-service training program can improve critically ill patients' safety during intra-hospital transportation.
The Intensive Care Unit is a unique hospital setting, not only because of its physical environment but also because the patients are unstable and submitted to physical and psychological stresses. Delirium is a common disorder in Intensive Care Unit. It is still misdiagnosed by healthcare providers and is treated inappropriately or neglected. Nursing care for delirium includes regular assessment and monitor frequently for at risk patients. The present study aimed to evaluate the effect of educational program on nurses" knowledge, practice and clinical outcome regarding delirium among critically ill patients. The study was conducted at Intensive Care Unit at Tanta University Hospital. It included 40 critical care nurses working in the previously mentioned setting. Also, 40 critically ill patients were used in this study. They divided into two equal groups. 20 critically ill patients were evaluated before implementation of educational program. Another 20 critically ill patients were evaluated after implementation of educational program. Three tools were used in the study for data collection. Instrument 1 is a structure questionnaire sheet. It was divided into two parts; Part A: nurses' socio demographic assessment sheet. Part B: nurse"s knowledge assessment sheet, Instrument 2: Observational Checklist and Tool 3: Intensive Care Delirium Screening Checklist (ICDSC). The main results of this study were that, the majority of the studied nurses have good knowledge after educational program. Also, the majority of studied nurses (97.5%) have poor score in their practice pre educational program compared to 77.5% % of them had good score in their practice post test. Also, about 30% of pre test group had delirium at 5th, 6th, and 7th days of admission compared with 10% in the post program group. The study recommended that In-services training program should be done continuously for updating critical care nurses" knowledge and practices regarding delirium detection and prevention. Also, Cognitive assessment in general and delirium assessment in particular should be incorporated into nursing education courses.
