http://reddi.unlam.edu.ar Pág: 2Este artículo se realiza en el marco de una investigación que tiene por objetivo optimizar un Sistema de Recuperación de Información, de desarrollo propio, mediante implementar y evaluar distintos algoritmos secuenciales y paralelos para resolver eficientemente la Descomposición de Valores Singulares. Dicho proceso comienza con la reducción de la matriz inicial a la forma bidiagonal. Estudios demuestran que la bidiagonalización puede consumir más del 70% del tiempo total del proceso. Por ello, como trabajo preliminar se han estudiado distintos métodos de bidiagonalización y se ha implementado un algoritmo basado en las transformaciones de Householder. El mismo se ha planteado con la suficiente flexibilidad como para ser adaptado fácilmente a otros algoritmos alternativos con el fin de realizar futuras implementaciones en arquitecturas paralelas, en particular las basadas en unidades de procesamiento gráfico.
Introducción: Nos hemos propuesto realizar un análisis regional en cuanto al abordaje diagnóstico y terapéutico de todos los tipos de síndrome coronarios, siendo este registro el primero en abordar aquellos que cursan sin enfermedad epicárdica significativa. Es importante conocer la realidad actual en la que nos encontramos debido a los constantes cambios de paradigma a los cuales nos vemos sujetos y a la amplia variedad de modalidades diagnósticas existentes en este tipo de síndromes, así como también la forma en que adaptamos la información disponible al medio en el que nos encontramos. Material y métodos: Se registraron en forma prospectiva pacientes hospitalizados por síndrome coronario agudo en 15 centros de Argentina con servicio de unidad coronaria y hemodinamia disponible las 24 horas. Resultados: Se incluyeron 984 pacientes consecutivos, de los cuales la mayoría presentaron enfermedad epicárdica significativa, siendo un 22.2% AI, 39.1% IAMSEST y 24.1% IAMCEST. Por otro lado el 4.1% se presentó como IAM de tipo 2, 1.2% como miocarditis. 0.7% como Takotsubo y 8.6% como MINOCA. La mediana de edad fue de 66 años (P25-75 56.5-74) con un 75.3 % de sexo masculino. El manejo inicial fue invasivo en el 84%, con una tasa de enfermedad coronaria significativa del 76.5% . En cuanto a la evolución intrahospitalaria, las complicaciones isquémicas más relevantes fueron el re IAM con una tasa de 2.84%, angina recurrente 2,4%, angina post infarto 2% y trombosis intra stent 0.5%. El porcentaje de eventos hemorrágicos totales fue de 4.4% y la mortalidad intrahospitalaria total fue de 3.76% . Conclusiones: El registro multicéntrico ReSCAR representa una primera aproximación del conjunto conformado por el amplio espectro de pacientes con injuria miocárdica como evento final independientemente de la etiología responsable de la misma. Brinda un primer panorama acerca de la elección del abordaje diagnóstico y terapéutico de aquellos síndromes coronarios sin enfermedad epicardica así como una aproximación pronóstica de los mismos. Además nos aporta información actualizada acerca de modalidades terapéuticas y pronóstico en pacientes con IAMCEST y una actualización sobre el abordaje terapéutico invasivo y antitrombótico en aquellos pacientes con SCASEST.
