A hiperostose esquelética idiopática difusa (DISH) é uma condição sistêmica caracterizada pela presença de pontes ósseas na coluna anterolateral, tem uma predominância no sexo masculino e pico de incidência a partir da quinta década de vida. Apesar de sua etiologia ser incerta, alguns fatores podem influenciar no seu desenvolvimento tais como: idade avançada, obesidade, hipertensão, aterosclerose, diabetes mellitus e sexo masculino. Na DISH, o paciente tem uma pré-disposição até quatro vezes maior de fratura vertebral, devido a incapacidade de redistribuição da energia de impactos para os tecidos moles, que se encontram calcificados. O tratamento de escolha para tal patologia é essencialmente cirúrgico e representa a principal escolha na maioria das vezes. Este relato tem como objetivo descrever a abordagem diagnóstica e o manejo terapêutico de um paciente com DISH tratado conservadoramente.
