Introdução. O processo de reabilitação de pacientes afetados pelo traumatismo cranioencefálico (TCE) possibilita uma melhor recuperação, tanto motora quanto funcional, além de promover qualidade de vida em pacientes, porém não há um consenso sobre quais modalidades de tratamento fisioterapêutico podem ser empregados. Objetivo. Realizar uma revisão sistemática para avaliar as modalidades fisioterapêuticas que reestabeleçam a função de forma mais efetiva no tratamento de pacientes com TCE. Método. Este é um estudo de revisão sistemática, cuja coleta foi realizada nas bases de dados MEDLINE (via PubMED) e na PEDro entre novembro de 2021 e maio de 2022. Para as buscas foram utilizados os seguintes termos de busca “traumatic brain injury” AND “physiotherapy”; na Physiotherapy Evidence Database (PEDro), acrescenta-se o preenchimento dos campos de pesquisa avançada disponíveis no portal. Foram incluídos apenas ensaios clínicos controlados aleatorizados completos. Os artigos selecionados foram avaliados de acordo com a Escala PEDro, a fim de quantificar a qualidade metodológica dos estudos a partir dos critérios de avaliação da referida escala. Resultados. Foram encontrados 233 artigos, onde 223 foram excluídos e 10 artigos restaram para serem analisados e tabulados. Conclusão. O atendimento fisioterapêutico baseado em exercícios foi eficaz em reestabelecer a função dos pacientes. Entretanto, os estudos indicam uma grande diversidade de condutas empregadas, não havendo superioridade de um tratamento em relação a outro.
