Resumo Avaliar e correlacionar o estado funcional (EF) e a força muscular (FM) em adultos jovens, adultos e idosos em Unidade de Terapia Intensiva (UTI). Coorte prospectiva com 48 pacientes avaliando EF por meio do Functional Status Score for the Intensive Care Unit (FSS-ICU) e FM pelo Medical Research Council Sum-Score (MRC-SS) e força de preensão palmar (FPP), no despertar e na alta da UTI. Dados analisados pelos testes Kruskall-Wallis, Qui-quadrado, Wilcoxon e Correlação de Sperman. Na comparação do despertar em relação à alta, o EF e o MRC-SS foram maiores na alta em todos os grupos, com menor ganho nos idosos. A FPP aumentou bilateralmente na alta, exceto no grupo de adultos, pois apresentavam valores maiores no despertar. Na comparação dos grupos, o FSS-ICU foi maior nos adultos no despertar e alta, e a FPP-D menor nos idosos. O EF apresentou forte associação com a FM em adultos jovens e adultos, pois ambos apresentam progressiva melhora durante a permanência na UTI. Os idosos apresentam menor ganho do estado funcional, além de apresentarem menor força muscular desde o despertar. Estas variáveis apresentam importante associação apenas nos adultos jovens e adultos, considerando a natureza multifatorial do processo de adoecimento e incapacidade nos idosos.
Introdução: O Sars-CoV-2, identificado como agente causador da Covid-19, disseminou-se rapidamente, fazendo com que a Organização Mundial da Saúde (o Ministério da Saúde) definisse a situação como pandemia. Para conter sua disseminação, o Brasil buscou adotar as recomendações do Ministério da Saúde, criando o Plano de Contingência Nacional para Infecção Humana (PCNIH). Objetivo: caracterizar a evolução das ações de políticas de saúde pública no enfrentamento da Covid-19, o avanço da doença nas regiões brasileiras e o impacto no SUS. Métodos: Trata-se de uma revisão integrativa, realizada nas bases de dados: PubMed, SciELO, ClinicalKey e nos principais sites governamentais, no período entre dezembro de 2019 e 16 de maio de 2020. Resultados: O PCNIH foi publicado em fevereiro de 2020 e estabeleceu as estratégias e as comissões de enfrentamento. Foram encontradas 37 portarias/resoluções e cinco comitês diretamente relacionadas ao PCNIH. Cerca de 10 medidas e ações para disseminação do coronavírus foram encontradas. No entanto, observou-se que houve a disseminação do vírus em todo o território nacional. Visando suprir essas demandas, o governo federal buscou investimento de leitos, contratação e capacitação de profissionais da saúde, ampliação dos atendimentos da atenção básica e canais de atendimento eletrônico. Conclusão: Apesar da confecção do PCNIH, observou-se que houve dificuldades na implementação das medidas encontradas para conter a disseminação do vírus em função da diversidade geopolítica brasileira. Assim são necessárias a adoção de medidas governamentais uniformes, que estimulem a população a segui-las, tendo em vista que a polarização política faz com que haja rupturas no engajamento contra a Pandemia.
Objective: to identify risk factors related to overactive bladder syndrome. Method: a cross-sectional study was performed with elderly women (>60 years) from the community of Ceilândia, in the Distrito Federal, Brazil, with or without symptoms of OBS, who were evaluated through interviews and questionnaires. The clinical and sociodemographic variables analyzed were: age; body mass index (BMI); parity, schooling, previous abdominal and urogynecologic surgeries, physical activity, smoking, constipation, systemic arterial hypertension (SAH), diabetes mellitus; depression and anxiety. The questionnaires applied were the Overactive Bladder Awareness Tool (OAB-V8), the Geriatric Depression Scale and the Beck Anxiety Scale. Data were analyzed descriptively. Binary logistic regression was used to evaluate the significant associations between the independent variables and the outcome of interest. Risk ratios were calculated for each independent variable with 95% confidence intervals. Result: A total of 372 volunteers were recruited, 292 of whom were eligible. Of these, 172 were allocated to the case group (58.9%) and 120 (41.1%) were control subjects. The two groups were homogeneous between one another. There was a high prevalence of OBS in the study population and significant differences for the variables presence of SAH, abdominal surgery and pelvic surgery, with the case group presenting a higher frequency of these events. In multivariate analysis, it was observed that an active sexual life reduces the chance of having OBS by 70.8%, while urogynecologic surgeries increase this risk 3.098 times. Conclusion: In univariate logistic regression analysis, BMI, SAH, a previous history of abdominal and urogynecologic surgery, number of abortions and the presence of symptoms of depression and anxiety, were found to be factors associated with OBS.
