A atenção farmacêutica é o conjunto de ações, realizada pelo farmacêutico, em colaboração com outros profissionais de saúde, que visam promover o uso racional dos medicamentos e a manutenção da efetividade e segurança do tratamento. A hipertensão é uma doença cardiovascular de alta prevalência entre a população, caracterizando um fator de risco significativo de morbidade e mortalidade. E diante dessa problemática, diversos pacientes portadores de hipertensão a partir do conhecimento empírico utilizam plantas medicinais para o tratamento. Porém, o mau uso de plantas medicinais, independente da origem, pode provocar problemas por exemplo: alterações na pressão arterial, interações medicamentosas, danos ao sistema nervoso central, fígado e rins, que podem levar a internações hospitalares e até mesmo a morte dependendo da forma de uso. Diante dessa problemática é de grande relevância a atenção farmacêutica no uso de plantas medicinais para evitar possíveis efeitos indesejáveis. Objetivou-se através da literatura identificar e relatar a importância da atenção farmacêutica no uso de plantas medicinais em pacientes portadores de hipertensão arterial sistêmica. O presente trabalho trata-se de uma revisão da literatura com consulta nas bases de dados SciELO, PubMed, LILACS e Google acadêmico. E durante a pesquisa foram selecionados 44 artigos nos idiomas português e inglês, publicados nos últimos cinco anos e também foram escolhidas 10 espécies de plantas medicinais que contém interações medicamentosa e potencial anti-hipertensivo. Diante dos resultados apresentados conclui-se que a utilização das plantas medicinais no tratamento da hipertensão arterial sistêmica, quando orientada e acompanhada pelo profissional farmacêutico é bastante seguro e eficaz.
