Identificación de microorganismos asociados con rinosinusitis crónica en pacientes adultos con inmunodeficiencia común variable Resumen ANTECEDENTES: la prevalencia de rinosinusitis crónica en pacientes adultos con inmunodeficiencia común variable (IDCV) es de 52%. Los pacientes con esta enfermedad tienen mayor frecuencia de rinosinusitis crónica, enfermedad inflamatoria que afecta a la mucosa de uno o más senos paranasales y la cavidad nasal.OBJETIVO: identificar los microorganismos de secreción del meato medio obtenida por endoscopia asociados con rinosinusitis crónica en pacientes adultos con inmunodeficiencia común variable (IDCV). MATERIAL Y MÉTODO:estudio descriptivo, transversal, que incluyó a pacientes adultos con inmunodeficiencia común variable, de quienes se obtuvo una muestra vía endoscópica de secreción del meato medio de ambas fosas nasales, que se envió a cultivo para bacterias aerobias, anaerobias y hongos. Se obtuvo consentimiento informado de todos los pacientes.RESULTADOS: se estudiaron 29 pacientes: 18 mujeres y 11 hombres, con edad promedio de 40±13 años. Los resultados obtenidos fueron: 2 muestras de pacientes no tuvieron desarrollo microbiano, 24 tuvieron desarrollo de bacterias aerobias, en 3 casos hubo crecimiento fúngico sin desarrollo de bacterias anaerobias. CONCLUSIONES
Antecedentes: Los abscesos profundos de cuello pueden ocasionar complicaciones letales. Se diagnostican por exploración física, y la tomografía contrastada es el método de referencia. No existen estudios de asociación del signo de Moore con infecciones del espacio retrofaríngeo. Objetivo: Determinar la utilidad del signo de Moore en el diagnóstico de absceso profundo en el espacio retrofaríngeo. Método: Estudio observacional, transversal y analítico, de pacientes con absceso profundo de cuello, del 1 de mayo de 2019 al 30 de agosto de 2021, con reporte de signo de Moore. Resultados: Se incluyeron 87 pacientes, de los cuales 49 (56.3%) eran de sexo masculino (p = 0.45). De los que desarrollaron complicaciones, el 77.8%, tenían el signo de Moore negativo (p = 0.001). De los que ingresaron a la unidad de cuidados intensivos, el 72% tenían negativo el signo de Moore (p = 0.001). La sensibilidad de la ausencia del signo con afección del espacio retrofaríngeo fue del 95.4%, y la especificidad del 86.3%. Por regresión logística se encontró que aquellos con afección del espacio retrofaríngeo tienen 467 veces más posibilidades de presentar signo negativo (p < 0.05). Conclusiones: La presencia de un absceso en el espacio retrofaríngeo se asocia a complicaciones y peor pronóstico. La evaluación del signo de Moore puede ser una herramienta útil para sospechar compromiso de ese espacio.
Antecedentes: la inmunodeficiencia común variable es la inmunodeficienciahumoral más frecuente en adultos. La frecuencia de infecciones de las vías aéreas, como rinosinusitis crónica, se reporta en 75% en estos pacientes, en comparación con 2 a 16% de la población general. La rinosinusitis crónica es una enfermedad inflamatoria que afecta a la mucosa de uno o más senos paranasales y la cavidad nasal.Objetivo: conocer la prevalencia de rinosinusitis crónica, su localización y severidad tomográfica en pacientes adultos con inmunodeficiencia común variable.Material y método: estudio transversal, descriptivo y observacional, que incluyó 21 pacientes adultos con inmunodeficiencia común variable. A todos los pacientes se les realizó tomografía computada de la nariz y de los senos paranasales, evaluada por un otorrinolaringólogo. La severidad de la rinosinusitis crónica se evaluó con la clasificación de Lund-Mackay.Resultados: de los 21 pacientes, 15 (71.5%) eran mujeres; la edad promedio fue de 40 años (± 13); 12 pacientes (52%) padecían rinosinusitis crónica, de acuerdo con la clasificación de Lund-Mackay, 2 de ellos (17.8%) tenían rinosinusitis crónica severa, 3 pacientes (27.7%) moderada y 7 pacientes (54.5%) leve. La localización más frecuente fue el seno maxilar bilateral, en 7 pacientes (33%), las celdillas etmoidales anteriores y posteriores en 6 pacientes (26%), el seno maxilar unilateral en 4 pacientes (19%), el seno frontal y esfenoidal, en igual proporción (3 pacientes, 13%); se encontraron las siguientes variantes anatómicas: desviación septal obstructiva (n=6, 28.5%), hipoplasia bilateral de los senos maxilares (n=2, 10%), concha bullosa unilateral (n=2, 10%), tabiques en el seno maxilar unilateral (n=3, 14.2%) y celdillas de Agger Nasi neumatizadas (n=1, 4.7%).Conclusiones: 52% de los pacientes con inmunodeficiencia común variable tenía rinosinusitis crónica, 58% de severidad leve, la principal localización fue el seno maxilar bilateral (33%). Debido a la prevalencia de rinosinusitis crónica en pacientes adultos con inmunodeficiencia común variable, debe realizarse tomografía computada a todos los pacientes para establecer el diagnóstico y tratamiento oportunos.
