Pseudomonas aeruginosa is one of the most common bacterium with a broad spectrum of human-associated infections. It is intrinsically resistant to many antimicrobial drugs, making carbapenems crucial in clinical management. The emergence and dissemination of carbapenemases among P. aeruginosa clinical isolates is a serious public health concern as it limits the options for the treatment of bacterial infections. Here, we described the molecular and epidemiological characteristics of 28 carbapenem-resistant P. aeruginosa strains isolated from patients hospitalized in an intensive care unit (ICU). The antimicrobial susceptibility of carbapenem-resistant P. aeruginosa strains was determined by broth microdilution. The presence of resistance genes was evaluated by PCR and DNA sequencing. Additionally, alterations in genes encoding P. aeruginosa outer membrane proteins were analyzed by PCR as well as SDS-PAGE. Clinical characteristics of the patients and the economic impact of hospitalization on the public health system were evaluated. PCR amplification showed that the bla KPC-2 and bla TEM genes were identified in three isolates (11%) and bla SHV gene in two isolates (7%). Outer membrane profiles obtained by SDS–PAGE indicated that the OprD porin was either absent or was produced at very low levels. A PCR assay using opr D-specific primers failed to show the presence of mutations in this gene. P. aeruginosa strains were isolated from 28 patients, among whom 43% (12/28) had sepsis, 31% (9/28) had respiratory failure, and 31% (9/28) had systemic arterial hypertension. A high mortality rate (39%) was observed in these patients, with an average duration of hospitalization of 34.6 days and a median cost of 3.275 dollars per patient. The production of carbapenemase was not the main mechanism of resistance in these strains. All carbapenem-resistant P. aeruginosa were isolated from patients hospitalized in the ICU. Besides the high mortality rate, many patients remained hospitalized for several days, resulting in a high cost of hospitalization for the public health system. Therefore, the evolution of this resistance and its dissemination should be actively monitored among critically ill patients to improve their health conditions.
We estimated the prevalence of screening for prostate cancer in indigenous people in Brazil. We also studied how ethnicity, age, social conditions, lifestyle, and history of sexually transmitted infections are associated with altered prostate-specific antigen (PSA) values. This is a cross-sectional study with indigenous people, ≥ 40 years old, from Dourados reserve, Mato Grosso do Sul, Brazil. The patients underwent total PSA and rapid tests for syphilis, HIV, and hepatitis B and C. PSA values were compared with sociodemographic conditions, presence of urological symptoms, clinical data on sexually transmitted infections, lifestyle, and family history of cancer. Out of the 498 men invited to participate in the study, 31.53% (157/498) were ≥ 40 years old and were included. The mean (±SD) age was 54.75 (±11.23) years, and 78.3% (123/157; 95% CI: 0.71–0.84) of the population never underwent any preventive examination for prostate cancer. The mean PSA value was 0.081 ng/mL for the 157 participants, and 4.4% (7/157) had > 2.5 ng/mL and 1.9% (3/157) had values ≥ 4 ng/mL. Rapid tests for STIs showed that 5.73% (9/157) of the participants had syphilis and 0.64% (1/157) had HIV, and Hepatitis B and C virus infection. The results showed that most indigenous people ≥ 40 years never underwent any preventive examination for prostate cancer, and 4.4% had an altered PSA exam result. Future studies should assess the factors that hinder adherence to prostate cancer screening, as well as the existence of a pathophysiological correlation between the occurrence of prostate cancer and STIs.
A craniotomia se faz uma técnica terapêutica eficaz para doenças e lesões cerebrais, sendo a dor pós-operatória uma importante preocupação clínica nesse contexto, a qual atinge em torno de 86% dos pacientes submetidos à craniotomia. O presente estudo de revisão buscou avaliar novas evidências para o alívio da dor em pacientes submetidos à craniotomia, documentadas por meio de estudos clínicos e randomizados. Trata-se de uma pesquisa de revisão integrativa realizada por meio da base de dados PubMed, que levou em consideração os seguintes critérios de inclusão: ensaios clínicos e testes controlados e aleatórios; artigos publicados no último ano; que possuíam texto completo disponível e que abordassem acerca da analgesia em pacientes submetidos à craniotomia. Ficou constatado que a infiltração pós-operatória do couro cabeludo com bupivacaína pura reduz a intensidade da dor e a utilização de analgésicos opioides, sendo considerada uma técnica eficaz de controle da dor após uma craniotomia eletiva. Ademais, a combinação de ropivacaína com costicosteroides como o diprospan pode se tornar uma das estratégias utilizadas para controle da dor incisional em pacientes submetidos a craniotomia, conferindo à esta técnica a capacidade de promover alívio satisfatório da dor pós-operatória nesses pacientes. Por fim, foi verificado que, em comparação com a analgesia sistêmica, a analgesia regional pode auxiliar na redução da incidência e da gravidade da dor após uma craniotomia e a quantidade de analgesia de resgate usada em tais pacientes.
RESUMOEste estudo objetiva relatar a experiência obtida por acadêmicos do curso de Medicina da Faculdade de Ciências da Saúde da Universidade Federal da Grande Dourados (UFGD) na Estratégia Saúde da Família-Piratininga (ESF-Piratininga) do município de Dourados-MS. Utilizou-se a metodologia de aprendizagem baseada em problemas, que consiste em observar a realidade, definir de pontos-chave, teorização, hipótese de solução e aplicação à realidade. Foi vivenciada a rotina de visitas da ESF-Piratininga durante 4 dias. Os alunos observavam, ora os ACS, ora a médica responsável pela unidade, durante os procedimentos regulares, correlacionando os conhecimentos teóricos adquiridos na disciplina de Atenção à Saúde da Comunidade com a prática visualizada e pensando em uma possível intervenção que ajudaria no dia a dia com os trabalhos da equipe. Foi observado que a introdução alimentar a lactentes era uma problemática pertinente a ser abordada como projeto de intervenção, foi concretizada então a realização de uma oficina de papinhas, produção de folder e material audiovisual. Nesse processo, a visão inter e multidisciplinar é abordada de maneira a beneficiar a formação acadêmica médica generalista, expondo os alunos a inúmeras situações necessárias e benéficas para todos os agentes envolvidos. Observou-se a importância da inserção precoce dos acadêmicos de medicina na prática da atenção à saúde da comunidade, já que houve o reconhecimento do protagonismo no processo de ensino-aprendizagem e aguçou a busca do saber científico e acadêmico balizado não só em dados epidemiológicos e clínicos, como também em valores humanos e sociais.
