Реферат. Сравнение моделей оценки риска носительства мутации BRCA1 и BRCA2 у больных раком молочной железы. Рыбченко Л.А., Бычкова А.М., Скибан Г.В., Клименко С.В. Проведен анализ эффективности использования алгоритма BOADICEA и Манчестерской балльной системы для прогнозирования носительства мутаций BRCA1 и BRCA2 у пациентов с раком молочной железы, проживающих на территории Украины. Материалом исследования служили результаты клинического, иммуногистологического, патогистологического, генеалогического, молекулярно-генетического исследования 146 пациентов с раком молочной железы. Расчеты риска мутаций проводились с применением алгоритма BOADICEA и Манчестерской балльной системы. Площадь под кривой в общей группе больных при прогнозировании BRCA1 мутаций алгоритмом BOADICEA составила 0,86, Манчестерской балльной системой − 0,84, а при расчете комбинированного риска BRCA мутаций − 0,83 и 0,84 соответственно. Однако статистической разницы между площадями алгоритмов не было установлено (р>0,05), что свидетельствует об одинаковой дискриминирующей мощности исследуемых моделей. Лучшую чувствительность, специфичность, прогностическую ценность позитивного и негативного результатов алгоритма BOADICEA было достигнуто при 6% вероятности BRCA1 и 8% пороге BRCA1/2 мутаций. Манчестерская балльная система показала лучшие операционные характеристики при 6ти и 13-ти бальной вероятности мутаций BRCA1 и BRCA1/2 соответственно. Пациентам с вероятностью мутаций с такими порогами можно предлагать молекулярное исследование патогенных аллелей.
