Background: It is well known that primary hyperparathyroidism (PHPT) is one of the most common endocrine disorders. Precise preoperative adenoma localization is essential for increasing PHPT cure rate. Conventional localization techniques include neck ultrasound, 99m-Tc-sestamibi scintigraphy, and computed tomography (CT). However, all of these methods have limitations. 11 C-methionine positron emission tomography/computed tomography (PET/CT) combines both anatomical and functional modalities; it may be useful in terms of lowering the imaging procedures number and improving accuracy.Methods: A retrospective diagnostic accuracy study with sensitivity and specificity evaluation was conducted. We studied the data of 91 patients with PHPT, who were hospitalized at Almazov National Medical Research Centre. Medical records, lab results, and CT imaging of all patients were analyzed. All of them underwent ultrasound. 99m-Tc-sestamibi/99m-Tc-pertechnetate subtraction scintigraphy and CT were performed on 56 and 86 patients, respectively. Since 2020 11 C-methionine PET/CT has been performed on 45 patients. Then, minimally invasive parathyroidectomy (PTX) was carried out in all patients.Histological results were used as a benchmark in order to evaluate diagnostic accuracy of studied methods.Parathyroid adenoma or hyperplasia was confirmed in all patients. Multiple lesions were found in 5 patients.Nineteen lesions were ectopic. All patients with multiple lesions required at least 3 localization techniques, and 2 of them required 4. Results:The sensitivity of 11 C-methionine PET/CT was 98%, CT, 99m-Tc-sestamibi scintigraphy, and ultrasound showed sensitivity at 75%, 79%, and 67%, respectively. The estimated specificities of 11 C-methionine PET/CT, CT, 99m-Tc-sestamibi scintigraphy and ultrasound were 93%, 73%, 75%, and 70%, respectively.Conclusions: Our study showed that 11 C-methionine PET/CT has higher sensitivity and specificity than conventional techniques in a group of 19 patients. 11 C-methionine PET/CT may take a place in the imaging of parathyroid adenomas, it may replace CT and 99m-Tc-sestamibi scintigraphy while simultaneously
Стремительное развитие методов компьютерной томографии в отечественной радиологической практике приводит к росту как индивидуальных доз пациентов, так и коллективной дозы населения России от данного вида лучевой диагностики. Для успешного проведения мероприятий по снижению доз облучения пациентов необходимо разрабатывать и внедрять в практику методы субъективной (экспертной) оценки качества изображения. В данной работе представлена комплексная методика оценки качества КТ-изображений на примере диагностики очагов по типу «матового стекла» с использованием антропоморфного фантома органов грудной клетки и разработанных имитаторов очагов. Методика экспертной оценки включает в себя необходимость локализовать патологический очаг на серии КТ-сканов и оценить качество изображения по пятибалльной шкале Лайкерта в соответствии с представленными критериями. Методика была апробирована при определении потенциальных низкодозовых протоколов сканирования органов грудной клетки с использованием различных алгоритмов реконструкции. Результаты опроса десяти врачей-рентгенологов (экспертов) показали возможность существенного (вплоть до четырех раз) снижения эффективных доз пациентов за счет снижения экспозиции при проведении КТ-сканирования органов грудной клетки в выявлении очагов по типу «матового стекла» без значимого снижения качества изображения. При этом наилучшие результаты соответствовали использованию алгоритма модельных итеративных реконструкций. Результаты исследования будут использованы для клинической апробации на пациентах. Целью данной работы являлась оценка физико-технических параметров качества КТ-изображений, выполненных на низкодозовых протоколах сканирования органов грудной клетки. Материалы и методы. Работа выполнена с использованием фантома ACR CT PHANTOM для компьютерного томографа Philips Ingenuity 128. Оценка качества изображения осуществлялась по точности определения единиц Хаунсфилда, шума изображения, отношения контраста к шуму и пространственного разрешения. Результаты исследования показали, отсутствие достоверных различий между различными низкодозовыми протоколами. При этом наблюдалась слабая корреляция между результатами физико-технических параметров изображения и экспертной оценки качества изображения, что не позволяет принимать решение о внедрении низкодозовых протоколов в практику, основываясь только на оценке физико-технических параметров.
The number and contribution of high dose imaging modalities, computed tomography in particular, is rapidly increasing both in the 'Russian Federation and other developed countries. Maximal increase in the number of computed tomography examinations in Russia was observed in 2020 due to the full-scale application of this imaging modality for the diagnostics of the novel coronavirus infection COVID-19. The use of computed tomography for the examination of the chest for the pregnant women is associated with several issues. An internationally accepted approach for the provision of the radiation safety of the pregnant patients is the assessment of the absorbed dose in the fetus after each X-ray examination. However, there are no existingn approved methods for the assessment of the absorbed dose in the fetus in the Russian Federation. The aim of the current study was to assess the doses in the fetus for the pregnant women undergoing computed tomography of the chest due to the COVID-19 and to estimate the probability of the development of the deterministic effects for the fetus. The study was based on the collection of the parameter of Russian and international computed tomography protocols. Parameters of the international computed tomography protocols were collected viameta-analysis of the existing publications; Russian protocols — via data collection in computed tomography departments in St-Petersburg hospitals. Absorbed dose in the uterus of the female patient and effective dose for the fetus were calculated using NCICT 3.0 software for 8, 10, 15, 20, 25, 30, 35 and 38 weeks of pregnancy. The results of the calculations indicate the lack of significant differences between absorbed doses in the uterus and effective doses for the fetus for all stages of pregnancy. Maximal doses for the selected computed tomog raphy protocols were in the range of 0.5 mGy (mSv) for 8-25 weeks, 0.6 mGy (mSv) for 30 week, 1.4 mGy (mSv) for 35 week and 2.7 mGy (mSv) for 38 week. The threshold for the development of the deterministic effects equal to 100 mGy cannot be exceeded even for repeated (10-15) computed tomography chest scans. Hence, the use of computed tomography as the primary method of COVID-19 diagnostics and staging will not be associated with the development of deterministic effects in the fetus.
