Die Endoskopie ist in der Gastroenterologie [1], der Hals-Nasen-Ohren-Heilkunde [2-4], der Schä-delbasischirurgie [5] und der Neurochirurgie [6] sowohl zur Diagnose als auch während der Therapie bzw. bei Interventionen ein unverzichtbares bildgebendes System. Die Videoendoskopie und das HD-Bildformat verändern jedoch zunehmend auch die Arbeitsabläufe der Eingriffe selbst. Vom technischen Standpunkt aus gesehen handelt es sich bei einem Endoskop wie bei einem Mikroskop um optische "Aufsicht"-Systeme. Neuartige Produkte wie das "Vitom" von Karl Storz zeigen die Fusion dieser beiden bildgebenden Konzepte. Der Wunsch und die Idee, Endoskopie [7] und Mikroskopie [8, 9] mit der computergestützten Chirurgie [10] auf der Basis von Navigation und Robotik zu koppeln, ist bereits knapp 20 Jahre alt. Die ersten Systeme der navigierten und robotergeführten Mikroskopie, die bereits mit einer automatisierten programmierbaren Bewegung der Bildgebung, mit Autofokussystemen und mit augmentierter Bildinformation im Display bzw. mit direkter Patientenprojektion ausgestattet wurden, waren Mitte der 90er-Jahre das "SurgiScope" [11] und das "MKM" [12]. Haltesysteme und Füh-rungssysteme für das Endoskop sind noch älter [13]. Die ersten Jahre des neuen Jahrzehnts zeigten jedoch, dass sich die großen und schweren Robotersysteme nicht so erfolgreich in die Operationssäle integrieren ließen, wie ursprünglich erwartet. "AESOP" [14] und "EndoAssist" [15] setzten sich nicht durch. Bemerkenswerten wirtschaftlichen Erfolg hat, nach den ersten begrenzten Erfolgen in der Herzchirurgie [16] und der Viszeralchirurgie, seit wenigen Jahren das laparoskopische Robotersystem "DaVinci" in der Urologie und Gynäkologie erzielen können. "DaVinci" ist jedoch kein einfaches Assistenzführungssystem für ein Endoskop sondern ein kompletter Telemanipulationsarbeitsplatz für einen minimalinvasiven Chirurgen. Der Roboter hat einen Anschaffungspreis jenseits der 1-Mio.-Euro-Grenze und bringt ganz erhebliche Kosten für Disposables während jedes Eingriffs mit sich. Zusammenfassung ! In diesem Artikel werden mehrere neuartige Verfahren für die Navigations-und Roboterunterstützung der Endoskopie beschrieben. Es wird erläutert, dass die Grundlagen bereits vor mehr als 15 Jahren gelegt wurden, sich die Umsetzung in praxistaugliche Geräte aber als schwierig gestaltet. Im Beitrag stehen Kleinroboter für die starre Endoskopie im Kopf-Hals-Bereich und die Füh-rung von Feininstrumenten und Mikrokameras im Mittelpunkt. Die vorgestellten Systeme sind klinisch evaluiert worden und werden systematisch verbessert. Eine besondere Bedeutung wird zukünftig den instrumentenmontierten Mikrokameras zukommen. Abstract !
The study shows a significant reduction in the number of errors by using a new manipulator concept compared to the non-augmented human hand in an experimental setup. We observed a significant learning effect when subjects applied the micro manipulator, resulting in reduction of the time requirement while maintaining a constant number of errors.
