KasuistikWir berichten über eine 29-jährige Patientin (IIgr. Ip.), die sich mit dem Verdacht einer Extrauteringravidität in der 12. SSW vorstellte. Die vorausgegangene Schwangerschaft verlief unauffällig mit Spontanpartus am Termin zwei Jahre zuvor. Die stationäre Aufnahme erfolgte wegen des Verdachts auf eine Extrauteringravidität in der rechnerisch 12. SSW. Eine Woche vor der stationären Aufnahme wurde bei V. a. gestörte Schwangerschaft ambulant eine Kürettage durchgeführt. Die histologische Untersuchung des Abradats ergab keinen Anhalt für eine intrauterine Gravidität. Nur Decidua compacta, ein Arias-Stella-Phänomen, aber weder Plazenta noch Chorionzotten waren nachweisbar. Die klinische Symptomatik der Patientin war blande. Es bestand lediglich eine schmerzfreie leichte vaginale Blutung. Die gynäkologische Untersuchung ergab keinen pathologischen Tastbefund in den Adnexbereichen. Sonographisch fand sich ein leeres Cavum uteri, eine Extrauteringravidität lieû sich nicht darstellen.Die viermalig im Abstand von jeweils 2 Tagen durchgeführten b-HCG-Bestimmungen ergaben erhöhte Werte um 4000 mIU/ml.
EinleitungIn der gynakologischen Onkologie geh6rt die Hormonrezeptoranalytik seit einigen Jahren zu den Selbstverst/indlichkeiten bei der Analyse eines Tumors. Dies gilt insbesondere ftir das Mamma-und Endometriumkarzinom, da bei diesen Tumoren aus der Kenntnis des Steroidrezeptorbesatzes therapeutische Konsequenzen gezogen werden. Die Bedeutung der Steroidrezeptoren beim Ovarialkarzinom ist wegen der morphologischen Inhomogenit~it dieses Tumors nicht endgtiltig bekannt und findet in therapeutischen Strategien noch keinen Niederschlag.Maligne Tumoren des Gastrointestinaltraktes, die chirurgischerseits behandelt werden, werden beztiglich ihrer Rezeptoreigenschaft routinem~ig nicht tmtersucht.Wir berichten tiber eine Patientin, die zun/ichst wegen eines Magenkarzinoms operiert worden war und 1 Jahr sp~iter wegen Adnextumoren behandelt wurde.
