SUMMARY The outcome of an eight‐week period of additional occupational therapy, aimed at improving fine motor skills, was assessed by accelerometry and a rating scale based on the Erhardt Developmental Prehension Assessment. 10 children with cerebral palsy received their regular occupational therapy, while a similar group had an additional 30‐minute session each day. No difference was found between groups at the end of the study. The difficulties of designing this type of project are discussed and the assessment methods described. RÉSUMÉ Mesures subjectives et objectives pour étudier le résultat de traitements supplémentaires chez les enfants avec IMC Le résultat d'une période du huit semaines d'une rééducation ergothérapique supplémentaire visant à améliorer l'activité motrice fine a été appréciée par Developmental Prehension Assessment. 10 enfants IMC bénefiérent d'un traitement d'ergothérapie habituelle tandis qu'un groupe semblable bénéficiat d'une session supplémentaire de 30 minutes chaque jour. Aucune difference n'a été notée entre les groupes à la fin de l'étude. Les difficultés pour établir ce type de projet sont discutées et les méthodes d'évaluation sont décrites. ZUSAMMENFASSUNG Subjetive und objektive Beurteilung des Erfolgs einer zusätzlichen Therapie bei Kindern mit Cerebralparese Der Erfolg einer zusätzlichen Beschäftigungstherapie über den Zeitraum von acht Wochen, mit dem Ziel, die Feinmotorik zu verbessern, wurde mit Hilfe der Akzelerometrie und einer Einstufung nach der Erhardt Developmental Prehension Assessment beurteilt. 10 Kinder mit Cerebralparese erhielten ihre regelmäßige Beschäftigungstherapie, während eine entsprechende Gruppe täglich zusätzlich 30 Minuten behandelt wurde. Am Ende der Studie fand sich kein Unterschied zwischen den Gruppen. Die Schwierigkeiten der Planung eines solchen Projektes werden diskutiert und die Methoden der Beurteilung werden beschrieben. RESUMEN Medidas subjetivas y objetivas para estudiar el curso de una terapia adicionel en niños con parálisis cerebral Se evaluó el curso seguido durante un periodo de ocho semanas de terapia ocupacional adicionada que pretendia mejorar la motilidad fina. Se utilizó la acelerometría y una escala de proporción basada en el Erhardt Developmental Prehension Assessment. 10 niños con parálisis cerebral recibieron su terapia ocupacional habitual, mientras que un grupo similar recibia una sesión añadida de 30 minutos cada dia. No se halló ninguna diferencia entre ambos grupos al final de este estudio. Se discuten las dificultades para el diseño de este tipo de proyecto y se describen los métodos de evaluaciön.
SUMMARY An objective test of motor function would enable better evaluation of various treatment programmes for children with cerebral palsy. The use of tri‐axial accelerometry was investigated in 36 children with cerebral palsy and in 53 children without disabilities. Variables discriminated well between able‐bodied and disabled children, and there was a high level of retest reliability. The technique requires further development, but shows promise of providing the ability to gather objective data about the quality of motor function. RÉSUMÉ Test objectif de la qualité de la fonction motrice des jeunes IMC: étude préliminaire Un test objectif de la fonction motrice devrait permettre une meilleure évaluation des programmes de traitement variés pour les enfants I.M.C. L'emploi d'une accelerométrie triaxiale a été analysé chez 36 enfants I.M.C. et 53 enfants sans troubles. Les variables différenciaient bien les enfants sans et avec incapacité et il a été observé un taux élevé de fidèlité test/retest. La technique exige des perfectionnements à venir mais se montre prometteuse pour recueillir des données objectives sur la qualité de la fonction motrice. ZUSAMMENFASSUNG Objektiver Test zur Beurteilung der motorischen Funktion bei Kindern mit Cerebralparese: vorläufige Studie Ein objektiver Test der motorischen Funktion würde eine bessere Beurteilung der verschiedenen Behandlungsprogramme für Kinder mit Cerebralparese ermöglichen. Bei 36 Kindern mit Cerebralparese und 53 Kindern ohne Behinderungen wurde die Anwendung der triachsialen Accelerometrie untersucht. Die Variablen unterschieden sehr gut zwischen nicht Behinderten und Behinderten. Las variables discriminaban bien entre nifios capaces e incapaces y habia un alto nivel de fiabilidad test‐retest. La técnica requiere un mayor desarrollo, pero se muestra prometedor para proporcionar la capacidad de obtener datos acerca de la cualidad de la función motora.
SUMMARY This study evaluated the use of accelerometry to measure the quality of movement in children with cerebral palsy. Accelerometer scores based on a previously described test were correlated with scores on a newly developed clinical test shown to be both valid and reliable. Low correlations were obtained between clinical test scores and accelerometer scores. Although the accelerometer test has been shown to be effective in distinguishing between able‐bodied children and those with cerebral palsy, it is not sufficiently sensitive to distinguish between levels of movement quality in the latter population. The accelerometer may measure a different aspect of movement from that measured by the clinical test, and does not appear to be a valid test of the quality of movement in children with cerebral palsy. RÉSUMÉ Comparaison des mesures subjectives el objectives de la performance du mouvement chez l'enfant IMC Cette étude fait appel á l'accélérometre pour mesurer la qualité du mouvement chez l'enfant IMC. I. es scores d'accélérometrie basés sur un test antérieurement décrit étaient corréles avec les scores d'un nouveau test clinique qui s'est révéléà la fois valide et fidèle. De faibles corrélations ont été observées entre les scores au test clinique et les scores d'accélérométrie. Bien que le test d'accélérométrie se soil révélé efficace dans la distinction entre un enfant capable et un enfant IMC, il n'a pas montré de sensibilité suffisante pour permettre une distinction entre les niveaux de qualité de mouvement dans cette dernière population. l'accéléromètre peut mesurer un aspect différent du mouvement que celui apprécié par le test clinique, et il n'apparaît pas un test valide de la qualité du mouvement chez l'enfant IMC. ZUSAMMENFASSUNG Vergleich subjektiver und objektiver Parameter der Bewegungsfähigkeit bet Kindern mil Cerebralparese In dieser Studie wurde die Anwendung der Accelerometrie zur Beurteilung der Bewegungsqualität bei Kindern mit Cerebralparese umersucht. Accelerometrie‐Scores, basierend auf einem zuvor beschriebenen Test, wurden mit den Scores eines neu entwickelten klinischen Tests, der sich als gut und zuverlässig erwiesen hat, korreliert. Es fanden sich niedrige Korrelationen zwischen den klinischen Test‐Scores und den Accelerometrie‐Scores. Obwohl sich der Accelerometrie Test als geeignet erwiesen hatte, zwischen gesunden Kindern und Patienten mit Cerebralparese zu unterscheiden, ist er nicht empfindlich genug, graduelle Unterschiede der Bewegungsqualität bei den Patienten mit Cerebralparese aufzuzeigen.
