In children with unexplained respiratory symptoms, laryngopharyngeal reflux should be suspected. Therefore, until enough data on this issue in the literature accumulates, the history and the laboratory findings of the patients obtained from various techniques to document paediatric LPR should be evaluated together.
Celiac disease (CD) is one of the most common chronic disorders in childhood. Autoimmune and nonautoimmune disorders including dermatitis herpetiformis, type 1 diabetes mellitus, and autoimmune thyroiditis can be encountered associated with CD. Common hematologic manifestations of CD include anemia owing to iron, folate, or vitamin B12 deficiency. We report a case with CD associated with Evans syndrome of whom to our knowledge, is the first child to be reported in the literature.
We present a 10-month-old child with central pontine myelinolysis (CPM) secondary to chronic active hepatitis due to cytomegalovirus (CMV) infection. A total of 35 paediatric cases of pontine and/or extrapontine myelinolysis are reported and, to our knowledge, CPM secondary to CMV hepatitis in an infant has not been previously reported. The MRI findings are highlighted.
ÖZETAmaç: Gastrointestinal sistem malformasyonlu (GİSM) çocuklarda doğumsal kalp hastalıklarının sıklığını ve mortalite ile ilişkisini saptamak. Yöntemler: Hastanemizin, Çocuk Cerrahisi Kliniğinde GİSM nedeniyle izlenen ve kardiyolojik inceleme yapılan ardışık 242 çocuk hastanın kayıt-ları retrospektif olarak incelenerek, GİSM olan çocuk hastalarda doğumsal kalp defektlerinin (DKH) insidansı araştırılmıştır. Verilerin istatistiksel değerlendirmesinde Ki-kare testi kullanılmıştır. Bulgular: GİSM saptanan, yaşları 0 gün ile 15 yıl arasında değişen 242 hasta değerlendirildi. Hastaların 135'i (%55.8) erkek, 107'si kız (%44.2) idi. En sık görülen gastrointestinal malformasyonları anorektal malformasyon (%43.2), mide, ileum veya kolon atrezisi (%21) ve özofajiyal atrezi/ trakeoözo-fajiyal fistül (%18.3) idi. Hastaların %28.5'inde doğumsal kalp hastalığı saptanmış olup, en sık gözlenen anomaliler; olguların 31'inde (%44.9) atriyal septal defekt, 17'sinde (%24.6) ventriküler septal defekt, 5'inde (%7.2) patent duktus arteriyozus idi. GİSM nedeni ile ameliyat edilen doğumsal kalp hastalığı olan (%16.7) ve olmayan (%13.3) hastalar arasında mortalite açısından anlamlı fark saptanmadı (p>0.05). Sonuç: Gastrointestinal sistem malformasyonu saptanan hastaların erken dönemde kardiyolojik açıdan değerlendirilmesi gerektiği vurgulanmak istenmiştir. Methods: Two hundred and forty two consecutive children patients with GISM followed up in Pediatric Surgery Clinics of our hospital were examined for cardiovascular anomaly by the Department of Pediatric Cardiology, and the CHD incidence was investigated by examining the records of the patients retrospectively. Chi-square test was used for the statistical analysis of data. Results: Two hundred and forty two patients with gastrointestinal system malformations were included in the study. Of 242 patients, 135 (55.8%) were male and 107 (44.2%) were female, and their age range was 0-15 years. The most frequent GISM were anorectal malformations (43.2%), atresia involving stomach, ileum or colon (21%) and esophageal atresia/tracheoesophageal fistula (18.3%). Congenital heart defects were observed in 28.5% of the participants. The most frequent defects were as follows; atrial septal defect (31 patients, 44.9%) a, ventricular septal defect (17 patients, 24.6%) and patent ductus arteriosus (5 patients, 7.2%). There was no significant difference (p>0.05) in mortality rate in patients with CHD (16.7%) and without CHD (13.3%) undergoing operations for GISM. Conclusion: We would like to emphasize the importance of the earliest possible cardiological evaluation of all patients with gastrointestinal system malformations. Hypertrophic osteoarthropathy is a syndrome characterized by clubbing of the digits of the hand/foot, periosteal reaction and arthralgia or arthritis which is usually secondary to cyanotic congenital heart disease and chronic pulmonary infections. This syndrome rarely occurs in association with chronic liver disease in childhood. Here, we report on a child with biliary atresia who developed arthralgia and arthritis during follow-up and which was diagnosed as hepatic hypertrophic osteoarthropathy. It is emphasized that hypertrophic osteoarthropathy should be considered in the differential diagnosis of arthralgia and arthritis in children with long-standing chronic liver diseases, especially if finger clubbing is also present.
CHB does not have deleterious effects on height and weight. Although IFN treatment temporarily compromises weight gain and growth velocity, lamivudine does not have any additional adverse effect.
Niemann-Pick hastalığı (NPH), sfingomyelinaz enzim eksikliğine bağlı olarak vücudun çeşitli organlarında, özellikle retiküloendotelyal sistem ve santral sinir sisteminde, intraselüler sfingomyelin ve kolesterol birikimi ile karakterize, otozomal resesif geçişli, nadir görülen bir lizozomal lipid depo hastalığıdır [1]. Genetik, klinik ve biyokimyasal heterojeniteye sahip hastalığın altı tipi tanımlanmıştır: Tip A-Akut nöronopatik form, Tip B-Kronik visseral form, Tip C-Kronik nöronopatik form daha iyi tanımlanmış formlardır. Tip A tüm hastaların %85' ini oluşturur [1,2].Niemann-Pick hastalığında sıklıkla nörolojik ve gastrointestinal sistem bulguları, büyüme-gelişme geriliği, göz bulguları, nadiren de solunum sistemi, cilt ve kardiyak bulgular görülür. Tanı, sfingomyelinaz aktivitesinin tespiti ve Niemann-Pick hücrelerinin kemik iliği incelemesinde gösterilmesi ile konur [1,3]. Burada NPH tanısı alan iki farklı tipte (tip A ve B) olgu sunuldu. Olgu SunumuOlgu 1: 15 aylık erkek hasta, karın şişliği, yürüyememe, konuşamama şikayetleriyle başvurdu. Öyküsünden aralarında üçüncü derece akrabalık olan anne babanın ikinci çocukları olarak, miadında, hastanede, 3800 gr olarak, normal yol ile doğduğu, iki kez pnömoni nedeniyle hastanede yattığı öğrenildi. Kardeş ölümü ve ailede benzer hastalık öyküsü yoktu. Bir aylıkken karın şişliği başlamış, altı aylıkken karaciğer ve dalakta büyüme farkedilmiş ve gittikleri merkez tarafından tetkikleri yapılmış; alfa-1 antitripsin, ter testi, viral serolojiler, immünglobulin M-A-G-E, göz muayenesi, kemik iliği aspirasyonu incelemesi, idrar ve kan aminoasitleri ve tandem mass incelemeleri normal bulunmuş. Karaciğer biopsisinde hepatositlerde sekonder lizozomlar çok sayıda, geniş ve birleşmiş olarak izlenmiş ve hepatositlerde yoğun lipid damlacıkları görülmüş. Nöromotor gelişim öyküsünde; dört aylıkken ÖZET Niemann-Pick hastalığı, belirli dokularda, özellikle retiküloendotelyel hücrelerde, sfingolipidlerin biriktiği, konjenital lipidoz grubundan otozomal resesif kalıtılan bir hastalıktır. Klinik fenotip olarak akut neonatal formdan geç başlangıçlı erişkin forma kadar aralığı oldukça değişkendir. Karaciğer ve dalak büyüklüğü şikayetiyle kliniğimize başvuran Niemann-Pick hastalığı tip A ve tip B tanıları alan iki hasta sunuldu.
