Unser Aufsatz kommt zu dem Ergebnis, dass Hormonbehandlungen bei Jugendlichen mit GIK/GD zu Lasten der GKV möglich sind. Dieses Ergebnis steht im Einklang mit der Entwicklung in der medizinischen Wissenschaft, wonach transidente Menschen nicht deshalb krank sind, weil ihre empfundene Geschlechtsidentität nicht (vollständig) mit dem Zuweisungsgeschlecht übereinstimmt. Wir stellen den wissenschaftlichen Erkenntnisstand dar, klären die Unterschiede zwischen der ICD-10, der ICD-11 und dem DSM-5 sowie zwischen Diagnosen, medizinischen Indikationen und Leitlinien. Dabei sind die Indikationsregeln für Jugendliche durch die SoC-8 im September 2022 entschärft worden. Anschließend wird erläutert, dass Krankenbehandlungsansprüche nicht allein bei einer manifesten GD, sondern bereits bei einer GIK mit dem Risiko für die Entstehung einer GD gegeben sein können. Gleichwohl besteht regulatorischer Handlungsbedarf.
