We report on a large series of 1,275 patients who underwent outpatient vasectomy performed by a single urological surgeon within a 10-year period. In addition, the results of a prospective questionnaire-based study on 217 patients regarding their opinions and motivation, and the financial aspects of the vasectomy are discussed. The mean age was 37.0+/-5.9 years. A total of 98.4% of the patients had an uneventful postoperative course. The average duration of sick leave was 2.2+/-0.21 days. During the study period, the length of sick leave dropped significantly from 2.63 (1990/91) to 1.1 days (1998/99; P=0.001). Efficacy was documented with azoospermia rates of 96.6% and 98.5% in patients who presented for two and three sperm examinations, respectively. A vast majority of patients (94.6%) felt that the procedure should be covered by their health insurance, although 88.1% stated they would also pay by themselves. A minority of patients (0.7%: 9/1,275) subsequently considered vasectomy reversal. The wish to reverse the vasectomy was significantly associated with a younger patient age. In conclusion, outpatient vasectomy provides a safe and reliable form of contraception at low cost. Overall satisfaction in appropriately counselled patients is very high. Based on these findings, further attempts to propagate vasectomy as a timely form of contraception are medically and socioeconomically recommended.
EinführungMit dem Gesetz zur Verbesserung der Versorgungsstrukturen in der gesetzlichen Krankenversicherung (GKV-VStG) 1 werden die mit dem Vertragsarztrechtsänderungsgesetz (VÄndG) 2 und mit dem GKV-Wettbewerbsstärkungsge-setz (GKV-WSG) eingeleiteten Reformen im Bereich der gesetzlichen Krankenversicherung fortgeführt. Ziel des Gesetzes ist die "Sicherstellung einer flächendeckenden, bedarfsgerechten und wohnortnahen medizinischen Versorgung der Bevölkerung"; das Gesetz soll hierzu eine Weichenstellung vornehmen, gleichzeitig aber auch zu einer nachhaltigen und sozial ausgewogenen Finanzierung der GKV beitragen. Um das selbst gesetzte Ziel zu erreichen, sollen im vertrags(zahn)ärztlichen Bereich u. a. eine Anpassung der Rahmenbedingungen für die Bedarfsplanung und Niederlassung sowie eine Neuausrichtung der Vergütungs-regelungen vorgenommen werden.Der Gesetzgeber hat erkannt, dass die zuletzt eingeleiteten Reformen, die u. a. von dem Gedanken getragen waren, zur nachhaltigen Sicherung der Versorgung eine Liberalisierung der Rahmenbedingungen (im Anschluss an die Beschlüsse des DÄT in Bremen) und eine Umstellung der Vergütungsregelungen vorzunehmen, allein nicht ausreichend waren; jedenfalls wurde in der Praxis die Absicht, das Vertragsarztsystem zu entbürokratisieren, zu vereinfachen und transparenter zu gestalten (so die Begründung zu § 87 b SGB i. d. F. GKV-WSG) offensichtlich nicht erreicht.Nicht zuletzt die Einführung der Regelleistungsvolumina (RLV) hat vielen Ärzten -ob altersbedingt oder auch nur aus frustranem Erleben -die Teilnahme an der vertragsärztlichen Versorgung nachhaltig verleidet. Insofern liegen erste Entscheidungen der Sozialgerichte vor 3 . Die wohlmeinenden Absichten des Gesetzgebers, die Vergü-tung durch Vorabmitteilung von Regelleistungsvolumina (RLV) kalkulierbarer werden zu lassen, haben durch die Beschlüsse des Erweiterten Bewertungsausschusses und ihre Umsetzung genau das Gegenteil dessen erfahren, was damit bezweckt gewesen ist 4 . Wer dies bezweifeln möchte, sollte sich der Mühe unterziehen zu versuchen, den Regelungsinhalt der § § 87 ff. SGB V und der dazu ergangenen Beschlüsse des (Erweiterten) Bewertungsausschusses in ihrer vollen Bedeutung zu erfassen 5 .Ob die mit dem GKV-VStG beabsichtigte Flexibilisierung der Honorarverteilung auf regionaler KV-Ebene hieran etwas ändern wird, ist zurückhaltend zu bewerten, da die Kassenärztlichen Vereinigungen voraussichtlich übergangs-weise die RLV fortführen werden. Immerhin ist anzuerkennen, dass der Gesetzgeber die mit der RLV-Einführung eingeleitete Fehlentwicklung rückgängig machen will.Auch die Absicht, ein reibungsloses Ineinandergreifen von stationärer und ambulanter Versorgung zu gewährleis-ten, stellt sich vor dem zwangsläufig bestehenden Interessengegensatz zwischen den Versorgungsebenen in der Praxis häufig als reines Wunschdenken dar. Erkennbar wird dies nicht zuletzt daran, dass die Versorgungssektoren (ambulant/stationär) wechselseitig Leistungsinhalte und damit wirtschaftliche Ressourcen abgreifen, was als sektorüber-greifend...
