Prevalencia de sensibilización a alergenos en niños escolares con asma que viven en la zona metropolitana de Guadalajara Resumen ANTECEDENTES: la sensibilización a alergenos es dependiente de las circunstancias geográficas y climáticas de cada región; en consecuencia, identificar los agentes que más comúnmente sensibilizan a los niños con asma permite planificar medidas de prevención.OBJETIVO: determinar la prevalencia de sensibilización a alergenos en niños escolares con asma. MATERIAL y MÉTODO: estudio transversal y prolectivo al que se incluyeron niños de 6 a 14 años de edad que padecían asma, atendidos por primera vez en un servicio de Alergología. Los pacientes se incluyeron consecutivamente entre el 1 de enero de 2014 y el 31 de diciembre de 2014. Se estimó la frecuencia de sensibilización a alergenos.RESULTADOS: se incluyeron 186 niños, la edad media del grupo fue 9.1 ± 2.5 años; 104 (55.9%) de sexo masculino y 82 (44.1%) del femenino. La mediana de pruebas positivas fue de 5 y 4/186 (2.2%) estuvieron monosensibilizados. La categoría predominante de alergenos fue la de interiores (90.3%), luego los árboles (71.0%), al final los hongos (9.7%). De manera individual, los ácaros del polvo casero fueron los más predominantes entre los alergenos de interiores, seguidos por los epitelios; en los pólenes de árboles fueron los encinos (31.7%) y los fresnos (28.0%), en las malezas destacó el estafiate (21.5%), en las gramíneas Zea mays (18.3%) y en los hongos fue Cladosporium spp (6.5%).CONCLUSIONES: la alta prevalencia de sensibilización a ácaros y epitelios en niños con asma estimula a implementar medidas de control ambiental tendientes a contribuir al mejor control de la enfermedad. anTeCeDenTes Entre las enfermedades respiratorias que más afectan a los niños está el asma; su prevalencia mundial durante la edad escolar es de 9.4% y en la adolescencia de 12.6%. 1 Una gran proporción de esos pacientes son alérgicos a los alergenos que se encuentran en su entorno, esta cifra puede llegar a ser incluso de 80% de los casos.2 Entonces, la exposición y la frecuencia de sensibilización a los aeroalergenos harían posible explicar las variaciones observadas en la prevalencia del asma. Antecedentes: los estudios relacionados con los patrones de sensibilización a especies de aeroalergenos poco comunes son infrecuentes.Objetivo: determinar la frecuencia de sensibilización a pólenes de relevancia alergológica provenientes de la subclase Rosidae en pacientes con alergia respiratoria.Pacientes y método: estudio descriptivo y transversal en el que se incluyeron pacientes de manera consecutiva a un grupo con rinitis alérgica y a otro con asma. Se estudiaron mediante historia clínica, exploración física y reactividad cutánea a un panel de alergenos, que incluyó a las especies Prosopis, Schinus, Acacia y Eucalyptus. Se determinaron las frecuencias de sensibilización y sus intervalos de confianza a 95% (IC 95%) respectivos.Resultados: se estudiaron 104 pacientes con rinitis alérgica y 99 con asma; en ambos grupos la media de edad se ubicó en la cuarta década de la vida y hubo predomino de mujeres. En los pacientes con rinitis alérgica se observó la siguiente frecuencia de sensibilización: mezquite 26.2% (IC 95%: 17.7 a 34.6%), pirul 11.5% (IC 95%: 5.4 a 17.6%), eucalipto 6.7% (IC 95%: 1.9 a 11.5%) y acacia 3% (IC 95%: 0.3 a 6.3%). En los sujetos con asma la conducta fue la siguiente: mezquite 13.1% (IC 95%: 6.5 a 19.7%), pirul 7.1% (IC 95%: 2 a 12.2%), eucalipto 4 (IC 95%: 0.1 a 7.9%) y acacia 3% (IC 95%: 0.4 a 6.4%). Los dos grupos manifestaron un predominio de sensibilización por los fresnos y encinos; al final se encontraron eucalipto, pino y acacia.Conclusión: la capacidad de sensibilización de los árboles analizados, con excepción del mezquite, es menor.
Antecedentes: en población pediátrica, pocas veces se busca la sensibilización alérgica a insectos de los órdenes Hymenoptera y Culicidae.Objetivo: establecer la prevalencia de sensibilización a abeja, mosco y hormiga roja en niños con enfermedades alérgicas.Material y método: estudio retrolectivo y descriptivo, en el que se revisaron los registros de la reactividad cutánea a alergenos de abeja, mosco y hormiga en un servicio de Alergia Pediátrica de enero de 2009 a diciembre de 2011. Se estimó la frecuencia de sensibilización y sus respectivos intervalos de confianza de 95% (IC95%).Resultados: se incluyeron 613 pacientes, 41% era del sexo femenino; la edad media del grupo fue de 6.2±3.7 años. La sensibilización alérgica al mosco fue de 15% (IC95%: 12.4 a 18.1), a abeja de 14% (IC95%: 11.4 a 16.9) y a hormiga roja de 6% (IC95%: 6.0 a 6.8); por sexo, la frecuencia no difirió significativamente. Los niños sensibles a las abejas o a las hormigas rojas tuvieron mayor edad que los no sensibilizados: 7.1 vs 6.0 años (p=0.011) y 7.4 vs 6.1 años (p=0.042), respectivamente. Durante las estaciones de otoño e invierno predominó la sensibilización a la hormiga roja.Conclusiones: hasta nuestro conocimiento, éste es el primer informe en México relacionado con la frecuencia de sensibilización a abejas, moscos y hormigas rojas realizado en niños. Además, nuestros resultados sugieren que se debe identificar de manera habitual la sensibilidad a estos insectos.
