Background:Cutaneomeningospinal angiomatosis or Cobb syndrome is a rare, not well understood phacomatosis that features metameric cutaneous and spinal arteriovenous malformations (AVMs). The first case was described in Boston in 1915, and since then, few more cases have been reported in the English literature. No case was found to be from Argentina.Case Description:The authors present a 16-year-old boy with acute quadriparesis and respiratory failure who was diagnosed as Cobb syndrome and treated with microsurgery alone with very good results.Conclusion:Authors believe that microsurgery, alone or combined with embolization, should be the mainstay of treatment. They also acknowledge Harvey Cushing's contribution to the description of the syndrome and propose the syndrome to be renamed as Cobb–Cushing syndrome.
Objetivo:Describir nuestra experiencia en el manejo de las Mavs analizando las características clínicas de los pacientes y los resultados postoperatorios.Método de análisis:Realizamos un análisis retrospectivo de 52 pacientes admitidos en el Servicio de neurocirugía para manejo quirúrgico: La información de referencia incluyo síntomas al inicio, diagnostico de admisión, hallazgos neurológicos y hallazgos en estudio por imágenes tales como tomografía cerebral, IRM cerebral y angiografía por sustracción digital. Los hallazgos postoperatorios de interés fueron: Mortalidad, examen neurológico postoperatorio y complicaciones asociadas. Presentamos nuestro análisis estadístico.Resultados:Edad promedio: 37,7 años. Distribución: Hombres: 61,5%. Motivos de consulta más frecuentes: Cefalea 63,5%, evento hemorrágico 59,6%, convulsiones 26,9%. Localización: Supratentorial: 92,9%, Infratentorial: 7,2%. 30,8%, de las Mavs fueron grado 2 y grado 3 Cincuenta por ciento del total presentaron aneurismas, del total de la MAVs, 59,6% debutó con sangrado, 26,9% con Crisis Convulsivas y 13,5% con déficit neurológico. Recibieron tratamiento endovascular previo a cirugía 30,7%. Durante el postoperatorio 23,1% presentaron mejoría clínica, 57,7% no presento modificación, 19,2% empeoraron en el postoperatorio. La mortalidad fue 13,5%.Conclusión:Creemos que el subgrupo de Mavs grados III a V representan una entidad que requiere una compleja toma de decisiones dada la alta incidencia de aneurismas asociados que presentan y su asociación con eventos de sangrado. Nuestra mortalidad postoperatoria coincide con la bibliografía. Palabras clave, Mavs- aneurismas asociados- Acv hemorrágico- convulsiones.
En pacientes con diagnóstico de oligodendroglioma (OD) y oligoastrocitoma (OA) la presencia de la codelección 1p19q presenta valor pronóstico y predictivo y se encuentra presente en alrededor del 50% de los casos. La técnica FISH (fluorescence in situ hybridization) es el método estándar para su evaluación. La técnica CISH (chromogenic in situ hybridization) es una alternativa y fue validada en otras neoplasias. Se analizaron las características histológicas de los pacientes con diagnóstico de OD y OA y su correlación con la codelección 1p19q detectada por CISH. En 3 de los 24 pacientes analizados (13%) se encontró la presencia de la codelección 1p19q. Sólo encontramos la codelección en aquellos con diagnóstico de OD grado II. No fue encontrada la mutación en ningún caso en: OD grado III y OA grado II y III. La frecuencia de detección de la codelección (13%) es inferior a la descripta en otros estudios que utilizaron la técnica de FISH. Las posibles causas de esta discordancia pueden atribuirse a que la cantidad de pacientes es insuficiente para validar el método, a errores en el procesamiento del material o en su lectura, o bien a que el método de CISH no es coincidente con FISH.
Los neurocitomas son tumores intraventriculares poco frecuentes del sistema nervioso central. La mayoría de las descripciones son casos aislados. Se los ha descripto como tumores benignos de pronóstico favorable; sin embargo, en los últimos años han aumentado las descripciones de casos agresivos caracterizados por recurrencia local. El índice de proliferación Ki67 es utilizado como factor pronóstico de recurrencia, con riesgo aumentado si es > 2%. Se incluyeron pacientes con diagnóstico histológico de neurocitoma cerebral, desde 2004 a 2012. Se realizó análisis retrospectivo de datos. Los objetivos fueron analizar la frecuencia de recidiva y la asociación entre Ki67 y recidiva. Se incluyeron 8 pacientes. Media de seguimiento: 60 meses. Edad media: 23 años. Tratamiento quirúrgico inicial: sólo 3 con resección completa tumoral. Se determinó Ki67 en 7 y en 3 fue ≤ 2%. Presentaron recidiva tumoral 6 de 8 pacientes (75%). Mediana del tiempo a recidiva: 26 meses. Dos de 6 con recidiva tumoral fallecieron y uno se encuentra en cuidados paliativos. Dos de 3 con Ki 67 ≤ 2% presentaron recidiva y 3 de 4 con Ki67 > 2% presentaron recidiva. Los 3 pacientes con resección completa presentaron recidiva. Se detectó una alta frecuencia de recidiva tumoral a diferencia de la literatura. Ki67 habitualmente utilizado como un factor de riesgo de recaída no ha demostrado serlo en este estudio.
