Resumen antecedentes:Coagulación intravascular diseminada (CID) e infiltración a la médula ósea son infrecuentes en el cáncer gástrico (CG) y revisten mal pronóstico. objetivo: Presentamos un caso con CG metastásico asociado a CID. Realizamos revisión sistemática de la literatura sobre la quimioterapia en este escenario. Caso reportado: Mujer de 37 años con adenocarcinoma gástrico poco diferenciado, metastásico y CID tratada con 5-fluorouracilo, ácido folínico (leucovorina) y oxaliplatino. Conclusiones: La combinación de CG y CID es infrecuente e impone reducida expectativa de supervivencia y un gran reto terapéutico. La quimioterapia muestra eficacia en el control del CG y la CID en comparación al mejor cuidado de soporte.Palabras clave: Cáncer gástrico. Coagulación intravascular diseminada. Quimioterapia. Disseminated intravascular coagulation associated with metastatic gastric cancer: One case report and systematic literature review abstract Background: Disseminated intravascular coagulation (DIC) and bone marrow metastases are infrequent in gastric cancer (GC) and confers a dismal prognosis. objective:We present a case of metastatic GC associated with DIC. We conducted a systematic review of the literature on the chemotherapy efficacy in this scenery. Reported case: A 37-year-old woman with metastatic poorly differentiated gastric adenocarcinoma, and DIC, who was treated with 5-Fluorouracilo, ácido folínico, oxaliplatino (FOLFOX).. Conclusions: The presence of DIC in asoociation with metastatic GC is uncommon and imposes a decrease in survival as well as a therapeutic challenge. Chemotherapy shows efficacy in the control of GC and DIC compared to the best supportive care.
RESUMENCon el abordaje endoscópico de la cirugía antirreflujo se incrementó el número de funduplicaturas en los últimos 25 años y, con ello, aumentó el número de funduplicaturas fallidas. La cirugía de refunduplicación presenta un índice de morbilidad alto. Objetivo: Analizar la experiencia de refunduplicaturas laparoscópicas y las ventajas que otorga el abordaje quirúrgico sistematizado. Material y métodos: Estudio de cohorte ambispectiva de 201 refunduplicaturas realizadas por un solo cirujano, con una misma técnica quirúrgica antes y después de las modificaciones a la misma, en un periodo de 20 años y cuatro meses. Se analizaron las características demográficas y perioperatorias. Resultados: Los síntomas más frecuentes fueron disfagia y reflujo. Se obtuvo tiempo quirúrgico promedio de 135.1 ± 10.2 minutos, estancia hospitalaria promedio de 2.6 ± 0.8 días y seguimiento promedio de 37.7 ± 4.7 meses. El neumotórax advertido fue la complicación más frecuente (n = 10, 4.9%). Las modificaciones efectuadas a la técnica disminuyeron la disfagia posoperatoria y el reflujo persistente (p < 0.05). Conclusiones: La sistematización de la técnica en la refunduplicatura reduce el tiempo quirúrgico y la morbilidad. La endoscopía transoperatoria, de vital importancia para llevar a cabo
