Bipolar disorder (BD) is a condition characterized by an alternation between phases of depression and mania. Its beginning is influenced by both genetic and environmental factors, causing neurological and psychological changes. In children, it is manifested through episodes of mood disturbance and pronounced psychosocial impairment, causing sleeping disorders and presenting a high rate of suicidal ideals. It has a very complex treatment, since you cannot neglect or exacerbate neither the episodes of mania or depression, taking them both in consideration when choosing the right medication and monitoring the symptoms and response to treatment. It is also a disease which has not a well defined process of etiology and pathophysiology. This study is a current literature review of a recurrent disease in psychiatry.
IntroductionCorticosteroids have either somatic and psychiatric side effects. Somatic adverse effects are well described while neuropsychiatric have received less attention. Psychiatric symptoms such as depression, psychosis and especially mania are common side effects of corticotherapy.Aims and objectivesDescribe a case of a 53 year old female patient, with no psychiatric history, who developed psychiatric symptoms during the treatment of an acute exacerbation of multiple sclerosis with large parenteral doses of corticosteroids. Three days after the onset of corticotherapy she developed increased energy, elevated mood, increased motor activity, racing thoughts, and diminished need for sleep. She subsequently developed grandiose and persecutory ideation and then feared her grandson was going to die. This lead to her admission to psychiatric unit of our hospital to address this behavioral problems. Her husband noticed that she had become “hyper” in the past when she received pulse corticosteroid therapy, but the most recent episode was by far the worst.MethodsRevision of the scientific literature through Pubmed, Embase and Psychinfo using search terms including corticosteroids, mania, depression, psychosis and mood.ResultsPrednisone was suspended and Risperidone 2 mg was begun for presumed steroid-induced mood disorder, and the patient became calmer and much less guarded over the next 2-3 days. After a week she recovered complete euthymic mood and reverted to normal functioning.ConclusionsThese data suggest that Risperidone is well tolerated and appears to be useful for mood disturbances associated with corticosteroid therapy. Controlled trials seem warranted to confirm these observations.
Introdução: O câncer de próstata é o quinto tumor maligno mais frequente no mundo. As taxas de incidência e de mortalidade por câncer de próstata variam grandemente entre os diferentes países, possivelmente em decorrência de fatores ambientais e das variações na probabilidade de eventos genéticos necessários para a progressão do tumor. O elevado número de internamentos por neoplasias malignas da próstata sugere ser reflexo da maior expectativa de vida da população masculina. No Brasil, a incidência dessa afecção tem aumentado consideravelmente. Sem considerar os tumores de pele não melanoma, o câncer prostático é o mais frequente em todas as regiões. Objetivo: Caracterizar o perfil epidemiológico das internações dos pacientes com neoplasia maligna da próstata no Ceará durante o ano de 2017. Método: Trata-se de um estudo descritivo e analítico, de caráter retrospectivo, baseado em dados secundários do Departamento de Informática do SUS (DATASUS). Entre janeiro e dezembro de 2017, foi contabilizado o número de internamentos por neoplasia maligna da próstata no Ceará, analisando a prevalência entre regiões macrorregiões de saúde assim como as variáveis raça/cor, faixa etária, caráter dos atendimentos e estabelecimento. Resultados: O Ceará apresentou um total de 1023 internamentos registrados por câncer de próstata no ano de 2017. Em relação às macrorregiões de saúde, há um predomínio do número de internamentos realizados em Fortaleza (82,01%), seguido por Sobral (8,60%), Cariri (7,42%), Litoral Leste (1,27%) e, por fim, Sertão Central (0,68%). Quanto à raça/cor, mais de 75% das vítimas são pardas entre 60 a 69 anos (40,17%). Analisando o estabelecimento de internação, o Instituto do Câncer do Ceará teve o maior índice (44,09%), seguido pelo Hospital Cura DARS (21,13%) e pela Santa Casa de Misericórdia de Fortaleza (17,80%). Das internações realizadas, 71,70% foram eletivas e 28,30% em caráter de urgência. Conclusão: Foi possível observar no estudo realizado que o Ceará acompanha a alta taxa de prevalência do país. O principal perfil epidemiológico das vítimas, corroborando com a literatura, foi idoso, pardo, principalmente das regiões litorâneas do Ceará. Logo, esses indivíduos merecem investimento da sociedade para que as internações e complicações da neoplasia sejam minimizadas.
Introdução: As neoplasias mais frequentes entre crianças e adolescentes são as leucemias, os tumores do sistema nervoso central e os linfomas. Os linfomas não-Hodgkin que ocorrem nas crianças correspondem a um grupo heterogêneo com diversos tipos histológicos, sendo mais comum o tipo Burkitt. Segundo dados do Instituto Nacional de Câncer (INCA), a taxa de mortalidade geral por linfoma não-Hodgkin cresceu 17% entre 2004 e 2013 (último dado disponível). Objetivo: Avaliar os índices de mortalidade por Linfoma nãoHodgkin em crianças e adolescentes (0 a 19 anos), no Brasil, entre 2009 e 2013. Método: Foi realizado um estudo quantitativo, descritivo, transversal, baseado em dados secundários do Ministério da Saúde e do INCA sobre casos de óbito por Linfoma não-Hodgkin, em todas as regiões do Brasil, entre os anos de 2009 e 2013. Resultados: A taxa de mortalidade média ajustada por idade para o Brasil foi de 2,70 por milhão de crianças e adolescentes no período analisado. Essa taxa foi mais expressiva no sexo masculino (3,66 óbitos por milhão) quando comparado ao feminino (1,70). A faixa etária com maior índice de mortalidade foi a de 15 a 19 anos, em ambos os sexos, com taxa de 4,45. Entre as regiões do país, as que apresentaram maiores taxas foram a Região Sul e a Nordeste (três por milhão), e as menores, Regiões Norte e Sudeste (2,50 por milhão). Conclusão: Neste estudo foi observado um maior risco de morte por Linfoma não-Hodgkin para crianças do sexo masculino, com idade entre 15 a 19 anos, além de residentes nas Regiões Sul e Nordeste do país. Sendo assim, é possível denotar que a faixa etária infantojuvenil possui taxa de prevalência que reflete o cenário brasileiro em que está inserida. Em suma, é possível observar a importância da análise das tendências de mortalidade por esse tipo de neoplasia hematológica, pois a mesma pode fornecer subsídios para avaliação de estratégias de detecção precoce voltadas para o grupo infantojuvenil.
