RESUMOA saúde bucal afeta a qualidade de vida e dignidade pessoal dos pacientes. Esse impacto parece ser mais grave em indivíduos medicamente comprometidos ou hospitalizados. O projeto intitulado "Atenção estomatológica em pacientes hospitalizados e portadores de doenças infectocontagiosas" desenvolveu atividades de atenção primária à saúde por meio de ações educativas e preventivas das doenças bucais nos indivíduos portadores de doenças infectocontagiosas que estavam em tratamento no Hospital Oswaldo Cruz (Curitiba/PR). Os indivíduos que participaram deste projeto foram submetidos a exame odontológico sob o auxílio de luz artificial nas enfermarias. Quando necessário, biópsias foram realizadas nos pacientes com lesões bucais sem diagnóstico clínico definitivo. 289 homens (65,38%) e 153 mulheres (34,62%) foram examinados. A média de idade dos indivíduos foi de 41 anos. As condições bucais mais frequentemente diagnosticadas nestes pacientes foram: doença periodontal, cárie, língua saburrosa e a candidose bucal. Este grupo de pacientes apresenta um risco elevado ao desenvolvimento de doenças bucais, especialmente as infecções oportunistas e as neoplasias. O diagnóstico precoce, o tratamento e a prevenção destas entidades promovem uma melhoria na saúde e na qualidade de vida destes indivíduos e o cirurgião-dentista desempenha um papel fundamental neste processo.
The aim of this study was to assess morphological and morphometrical alterations of oral squamous epithelial cells in type 1 HIV infected individuals. Oral smears were collected from tongue and buccal mucosa of 30 HIV infected (experimental) and 30 non-infected (control) individuals by liquid-based exfoliative cytology. The cells were morphologically analyzed and the nuclear area (NA), the cytoplasmic area (CA) and the nucleus-to-cytoplasm area ratio (NA/CA) were calculated. No morphological differences were found between the groups. The mean values of CA were decreased in tongue (P=.00006) and buccal mucosa (P=.00242) in HIV infected individual, while mean values of NA were increased (P=.00308 and .00095, respectively) in the same group. NA/CA ratio for experimental group was increased in both collected places, with P=.00001 (tongue) and P=.00000 (buccal mucosa). This study revealed that HIV infection was able to induce morphometrical changes on the oral epithelial cells.
Objetivo: O herpes-zóster é uma infecção viral causada pela reativação do vírus da varicela-zóster. Esta doença pode afetar pacientes imunossuprimidos e caracteriza-se por lesões papulovesiculares distribuídas na região inervada pelo nervo afetado, causando dores intensas. Este artigo relata o caso de um paciente do sexo masculino, leucoderma, 26 anos de idade, dependente químico, HIV positivo e com tuberculose ganglionar que procurou o Hospital Oswaldo Cruz (Curitiba/PR) tendo como queixa mal-estar geral, lesões na pele e mucosa oral, e linfadenopatia na região cervical. Discussão: O diagnóstico do herpes-zóster foi estabelecido por meio das características clínicas das lesões, e o tratamento estabelecido envolveu o uso de drogas antirretrovirais, analgésicos e antidepressivos. Conclusão: O cirurgião-dentista precisa estar atento às lesões papulovesiculares que acometem a pele e as mucosas de forma unilateral, acompanhadas de sin¬tomas de prurido e dor, pois elas garantem o diagnóstico do herpes-zóster.
Kaposi sarcoma is a vascular neoplasm composed of endothelium-lined vascular spaces and spindle-shaped cells. It often manifests with multiple vascular nodules on the skin and other organs. Kaposi sarcoma is rare in HIV negative patients and it is associated with HHV-8 infection. There are four types: classic, endemic, iatrogenic and HIV-associated. Patients with HIV/AIDS are also at increased risk of developing Kaposi sarcoma. Objective: The aim of this article is to present two cases of Kaposi sarcoma in the hard palate of HIV/AIDS male patients. Case report and Conclusion:Kaposi sarcoma should be considered in the differential diagnosis of any AIDS patient who presents with bluish, smooth, firm, nonpulsatile macule or nodule in the region of the hard palate. Development of Kaposi sarcoma in the oral cavity also has prognostic implications for untreated HIV patients, who are found to have higher death rates than patients affected only by cutaneous disease.
Herpes simplex labialis occurs by reactivation of herpes simplex virus type I, but infection with the virus type II can also lead to disease. Several factors, including exposure to intense sunlight, psychological stress or immunosuppression may trigger a recurrence. Objective: The purpose of this article is to report a case of herpes simplex labialis in an immunosuppressed patient. Case report: Male patient, 40 years of age, was admitted to the Hospital Oswaldo Cruz (Curitiba/PR) for presenting with dry cough and chest pain accompanied by fever, sweating and weight loss. The patient is a smoker, HIV positive and presented a case of pneumocystosis. The intraoral examination revealed a smooth tongue and erythematous candidiasis. Vesicular and crusted lesions were observed on the skin around the lips and vermilion. Considering these findings, diagnosis of recurrent herpes labialis was established. The patient was treated with acyclovir for 12 days when there was regression of the lesions. Conclusion: The dentist needs to be aware of cases of exacerbated recurrent herpes labialis that can be a strong indication of HIV infection/AIDS.
A Síndrome da Imunodeficiência Adquirida (AIDS) é uma doença infecto-contagiosa, causada pelo retrovírus HIV. A principal célula-alvo do vírus HIV é o linfócito TCD4 auxiliar. Uma característica patológica desse vírus é a diminuição progressiva da imunidade celular e o conseqüente aparecimento de infecções oportunistas e neoplasias malignas. As manifestações bucais da infecção pelo HIV são comuns e podem representar um importante valor diagnóstico da doença. Aproximadamente 60% dos indivíduos infectados pelo HIV apresentam essas manifestações. Portanto, a presença de alterações bucais sugestivas de imunodepressão deve buscar no paciente a possibilidade de infecção pelo HIV, ou alertar para queda imunológica naqueles que tem a doença. A terapia anti-retroviral diminui a carga viral e aumenta as células CD4+, resultando menor freqüência de lesões oportunistas. Quando estas aparecem durante o tratamento podem indicar falha terapêutica. Além da candidíase bucal, as manifestações bucais mais comuns em pacientes soropositivos são a leucoplasia pilosa, periodontite ulcerativa necrosante, herpes simples, herpes zoster e o sarcoma de Kaposi. Outras lesões, menos freqüente observadas são, histoplasmose, linfoma e o carcinoma de células escamosas. Tendo em vista tais aspectos, o presente trabalho tem como objetivo apresentar casos de pacientes HIV positivos internados em hospital apresentando lesões bucais.
