A polymyositis-dermatomyositis ritka, az idiopathiás infl ammatoricus myopathiák körébe tartozó szisztémás autoimmun betegség. Kórképei alapvetően eltérnek egymástól, csupán egy közös jellemzőjük van, a krónikus izomgyulladás. A diagnózis pontos felállítását tovább nehezíti, hogy az egyes kórképek gyakran nem típusos formában jelennek meg, ilyenkor a beteg további életesélyei azon múlnak, hogy gondolunk-e egyáltalán erre a betegségre. A szerzők 50 éves nőbeteg esetét mutatják be, akit háziorvosa myopathia gyanújával utalt kórházba. Az anamnézis, a klinikai kép, valamint az extrém laboratóriumi leletek alapján polymyositis-dermatomyositisre gyanakodtak, amelyet az immunológiai vizsgálatok egyértelműen igazoltak. A betegség kezdetekor alkalmazott nagy dózisú (1-2 mg/ttkg/nap methylprednisolon) kortikoszteroid-terápia jó stratégiának bizonyult, a beteg állapota gyorsan és látványosan javult. A kortikoszteroid által okozott szénhidrátanyagcsere-felborulást megelőzendő, az első kórházi naptól inzulinterápiára tértek át. A beteg gondozására a szerzők olyan interdiszciplináris teamet hoztak létre, amelyben az orvos és nem orvos szakemberek különös fi gyelmet fordítanak a testi betegségen túl a beteg pszichés és szociális létének megingására is, továbbá egyeztetett terv alapján végzik tevékenységüket. A polifázisos kórlefolyású betegnél a négy és fél éves követési idő alatt csupán két relapsus következett be, izomereje az ötfokozatú skálán -a két relapsust kivéve -általában 3 és 4 között volt, ami a beteg szoros követésének és az egyénre szabott terápia hatásosságának köszönhető. The disease exhibits high inter-individual variability, but chronic myositis is a common feature. As different manifestations often appear in atypical forms, establishing the precise diagnosis can be rather complicated. The prognosis and the patient's life expectations highly depend on whether the clinician considers this possibility in the diagnostic process or not. The authors present the case of a 50-year-old woman who was referred to hospital with suspected myopathy by her general practitioner. The history of the patient, the overall clinical picture and some marked laboratory abnormalities raised the possibility of polymyositis-dermatomyositis, which was unequivocally confi rmed by immunological tests. Drug therapy was started immediately with the administration of high dose corticosteroid (1-2 mg/kg/day methylprednisolone), which was found to be an effective strategy leading to fast and remarkable improvement in the patient's condition.
