Both a-DGPs showed higher sensitivity than anti-tTG IgA and AGA IgA in monitoring compliance with GFD, but a-DGP IgA+G seemed to perform better. a-DGPs did not outperform anti-tTG IgA for CD screening.
Introduzione. Gli stipiti di Staphylococcus aureus resistenti alle penicilline sintetiche (meticillina, oxacillina,) vengono contrassegnati con la denominazione MRSA (S.aureus meticillina resistente).Virtualmente tutti gli MRSA producono PBP (penicillin-binding protein) e PBP 2a (codificata dal gene mecA) che conferiscono resistenza a tutti gli agenti beta-lattamici disponibili. La rilevazione del gene mecA è il metodo fondamentale per la rilevazione di meticillino-resistenza. Roche Diagnostics ha sviluppato un kit Lightcycler in PCR Real-time per la rilevazione di tale gene. Scopo del lavoro è verificare l'applicabilità in un laboratorio di microbiologia delle nuove tecniche di biologia molecolare in real-time PCR rispetto al metodo tradizionale di Kirby-Bauer. Metodi. Sono stati analizzati 80 ceppi di S.aureus a partire da materiali diversi. L'analisi dei campioni è stata effettuata sia con lo strumento LightCycler (Roche) sia con il metodo tradizionale Kirby-Bauer. Risultati. Su 80 ceppi di S.aureus analizzati, 31 sono risultati resistenti alla meticillina e 32 hanno evidenziato la presenza del gene mecA. In un unico caso si è avuta una discordanza con positività per il gene MecA e negatività per la meticillino-resistenza. Complessivamente si è evidenziata una concordanza nel 98.8% dei casi e una discordanza nel 1.2% dei casi. Conclusioni. La metodica molecolare presenta vantaggi rispetto a quella tradizionale soprattutto per i tempi rapidi di effettuazione e, probabilmente, per la sensibilità. Inoltre la rapidità del test consentirà di attivare un processo tendente a ridurre l'incidenza di ceppi meticillino-resistenti e di meglio indirizzare la terapia antibiotica con significative ripercussioni per il paziente ed un migliore utilizzo delle molecole antibiotiche stesse.
