We concluded that it is possible to classify gait episodes of fallers and non-fallers in people with dementia during everyday life using accelerometry.

Background Spinal pain can result in unilateral atrophy of spinal muscles. Understanding side-to-side muscle activity during exercises can help clinicians address these deficits. This study determined if variations of bridging exercises specifically activated side-to-side trunk-muscle activity. Method Using surface electromyography on 20 healthy subjects (16 females), age 25.45 (±3.57) years, height 166 (±0.8) cm, weight 63.35 (±12.70) kg, muscle activity of left and right lumbar multifidus, iliocostalis lumborum thoracis (ICLT), rectus abdominis (RA) and external oblique (EO) was recorded during eight bridging exercises with stable, unstable and unilateral (left-leg off the ground) conditions. Results There were significant side-to-side differences in abdominal-muscle activity during all unstable exercises (mean difference range from 3.10 %MVC for RA to 9.86 %MVC for EO), and during all unilateral exercises (mean difference range from 3.22 %MVC for RA to 9.41 %MVC for EO), with the exception of RA in exercise-7. For the back muscles, there were significant side-to-side differences for multifidus during all unilateral exercises (mean difference range 5.35 %MVC to 11.72 %MVC), with the exception of exercise-5. None of the bilateral exercises (stable or unstable) produced side-toside differences for multifidus. For ICLT only exercise-3 produced significant side-to-side differences with a mean difference of 5.5 %MVC. In all cases where significant differences were noted, the left side of the muscles demonstrated the higher values. Conclusion The results suggest that specific exercises (unilateral/unstable) can target specific sides of trunk muscles.

Current fall detection technologies work well under laboratory conditions but it is still problematic to produce reliable results when these technologies are applied to real life conditions. Acceptance towards the sensors decreased after study participation although the system was generally perceived as useful or very useful.

The study shows that there is a considerably large difference between fall-detector evaluation studies in domestic environments and in laboratory trials.

Many societies across the world are confronted with demographic changes, usually related to increased life expectancy and, often, relatively low birth rates. Information and communication technologies (ICT) may contribute to adequately support senior citizens in aging societies with respect to quality of life and quality and efficiency of health care processes. For investigating and for providing answers on whether new information and communication technologies can contribute to keeping, or even improving quality of life, health and self-sufficiency in ageing societies through new ways of living and new forms of care, the Lower Saxony Research Network Design of Environments for Ageing (GAL) had been established as a five years research project, running from 2008 to 2013. Ambient-assisted living (AAL) technologies in personal and home environments were especially important. In this article we report on the GAL project, and present some of its major outcomes after five years of research. We report on major challenges and lessons learned in running and organizing such a large, inter- and multidisciplinary project and discuss GAL in the context of related research projects. With respect to research outcomes, we have, for example, learned new knowledge about multimodal and speech-based human-machine-interaction mechanisms for persons with functional restrictions, and identified new methods and developed new algorithms for identifying activities of daily life and detecting acute events, particularly falls. A total of 79 apartments of senior citizens had been equipped with specific "GAL technology", providing new insights into the use of sensor data for smart homes. Major challenges we had to face were to deal constructively with GAL's highly inter- and multidisciplinary aspects, with respect to research into GAL's application scenarios, shifting from theory and lab experimentation to field tests, and the complexity of organizing and, in our view, successfully managing such a large project. Overall it can be stated that, from our point of view, the GAL research network has been run successfully and has achieved its major research objectives. Since we now know much more on how and where to use AAL technologies for new environments of living and new forms of care, a future focus for research can now be outlined for systematically planned studies, scientifically exploring the benefits of AAL technologies for senior citizens, in particular with respect to quality of life and the quality and efficiency of health care.

Purpose Senior citizen falls are one of the highest-cost factors of healthcare within this population group. Various approaches for automatic fall detection exist. However, little is known about the seniors’ acceptance of these systems. The purpose of this paper is to investigate the acceptance of automatic fall detection devices as well as the technological commitment and the health status in community-dwelling adults with a predefined risk of falling. Design/methodology/approach Seniors with a risk of falling were equipped with either an accelerometer or an accelerometer with an additional visual and optical fall detection system in a sub-group of the study population for a period of eight weeks. Pre- and post-study questionnaires were used to assess attitudes and acceptance toward technology. Findings In total, 14 subjects with a mean age of 75.1 years completed the study. Acceptance toward all sensors was high and subjects were confident in their ability to handle technology. Medical assessments showed only very mild physical and no mental impairments. Measures that assured subjects privacy protection were welcomed. Sensor technology should be as unobtrusive as possible. Originality/value Privacy protection and uncomplicated use of the fall detection equipment led to high acceptance in seniors with high-technical commitment and good health status. Issues to further improve acceptance could be identified. Future research on different populations is necessary.
