Flow cytometry is a highly sensitive and specific approach for discriminating between normal and clonal plasma cells in multiple myeloma. Uniform response criteria after treatment have been established by the International Myeloma Working Group and the EuroFlow Group; however, the way in which flow cytometry data are reported has suffered from no collaborative or multicentre efforts. This study, involving 8 expert laboratories and 12 clinical hematology units of the Lazio region in Italy, aims to produce a uniform and shared report among the various Centres. From the pre-analytical phase to sample processing, data acquisition, analysis, and evaluation of the potential limitations and pitfalls of the entire process, the study reaches a final conclusion shared by laboratories and clinicians according to the most updated principles and recommendations. The aim was to identify the necessary data to be included in the clinical report by using multiple-choice questionnaires at every single stage of the process. An agreement of more than 75% of the laboratories was considered mandatory for the data to be included in the report. By ensuring the operational autonomy of each laboratory, this study provides a clear report that limits subjective interpretations and highlights possible bias in the process, better supporting clinical decision-making.
Riassunto Il crescente interesse per il dosaggio della vitamina D è aumentato negli ultimi anni in quanto legato a un'evidente correlazione tra bassi livelli sierici e numerose condizioni patologiche relative non solo al metabolismo osseo. L'aumento delle richieste cliniche e ambulatoriali ha quindi portato a una maggiore diffusione di diversi metodi automatizzati per la determinazione di questo analita. Presso l'Azienda Ospedaliera S. Giovanni Addolorata di Roma è stato condotto uno studio di confronto per il dosaggio di 25OHD su 93 campioni di siero utilizzando un metodo cromatografico (HPLC) e tre metodiche in chemiluminescenza sugli analizzatori Architect (Abbott Laboratories, Abbott Park, IL, USA), Centaur (Siemens Healthcare Diagnostics Inc., Deerfield, IL, USA) e Liaison XL (DiaSorim, Saluggia, VC, Italia). I tre saggi immunochimici hanno fatto osservare una certa variabilità nei risultati ottenuti rispetto al metodo cromatografico, differenziazione imputabile alla diversa fase preparativa dei metodi che ne determina una diversa sensibilità, influenzando quindi la quantificazione dell'analita d'interesse. The results of the immunochemical methods compared with HPLC showed some variability, differentiation due to different preparative phase of the methods that determine a different sensitivity, thus affecting the quantification of the analyte of interest. An accurate and precise quantification of vitamin D is significant to discriminate correctly patients who need an additional supplementation for deficiency effective and at the same time to avoid in patients with a normal dosage. In particolare, la 25OHD
altre cause di incremento della creatinina. L'evento avverso avviene nell'1-6% della popolazione generale e nel 30-50% dei pazienti a rischio. I fattori di rischio identificati, relativi al paziente, sono: l'insufficienza renale, il diabete mellito, l'esecuzione della procedura in urgenza/elezione, la presenza di palloncino intra-aortico per la contropulsazione aortica, l'insufficienza cardiaca congestizia, l'età, l'ipertensione arteriosa, un basso ematocrito, l'ipotensione arteriosa e una frazione di eiezione ventricolare sinistra inferiore al 40%. I più importanti fattori di rischio identificati, relativi al MdC, sono: un'alta osmolalità, un MdC ionico, la viscosità del contrasto e il volume di MdC somministrato. Nel passato sono stati descritti dei casi di nefropatia conseguenti alla somministrazione di MdC in pazienti affetti da mieloma multiplo o macroglobulinemia di Waldenström. Per questo motivo, in numerosi centri viene ricercata l'eventuale presenza di gammopatie monoclonali, prima di indagini radiologiche che prevedano l'uso del contrasto. Tuttavia, l'effettiva frequenza di nefropatie, in seguito alla somministrazione degli attuali MdC a bassa osmolalità, è molto rara. Pertanto, la presenza di gammopatie monoclonali non è solitamente inclusa tra i fattori di rischio. Alla luce di questo, la misura della creatininemia e dell'ematocrito sono i test di laboratorio raccomandati per le strategie di prevenzione della CIN. Qualsiasi altra indagine di laboratorio va riservata all'approfondimento che consegue alla valutazione clinicoanamnestica.
