Watson & Ballet and Vender staging systems are widely known for classifying SNAC wrist osteoarthritis. Despite of its day-to-day use, no assessment for its agreement was performed. To Evaluate the intra and interobserver agreement for these classification systems. Forty-eight posteroanterior wrist radiographs from patients with osteoarthritis due to scaphoid nonunion were evaluated at two occasions-in a 1 week interval-by five observers with different expertise-hand surgeons, hand surgery residents, orthopedic surgeons and orthopedic surgery residents. They rated osteoarthritis stages according to the above-cited systems. Kappa statistics were performed for measuring agreement. Unsatisfactory (Cohen's Kappa <0.5) agreement was found for all intra and inter observer measures for both systems. There was no clear correlation between expertise and kappa values. Unsatisfactory agreement was found for both classifications, despite the evaluator expertise. A rationale of a more reliable classification is needed.
OBJECTIVE:The aim of this study was to analyze the effect of high-energy extracorporeal shockwave therapy on tendon angiogenesis in the patellar tendons of rabbits. We sought to investigate whether different voltage and number pulses modify the angiogenesis pattern.INTRODUCTION:High-energy extracorporeal shockwave therapy is an option in the treatment of orthopedic diseases such as chronic tendonitis. Despite its potential clinical applicability, there have been few studies on this technique that examine both its clinical effectiveness and its effect on angiogenesis.METHODS:High-energy extracorporeal shockwave therapy was applied at the tibial insertion of the left patellar ligament in 30 rabbits that were separated into six groups that differed in terms of the voltage and number of pulses that were applied by high-energy extracorporeal shockwave therapy. The tibial insertion in the right legs of the animals was used as the control. After six weeks, we performed histological analysis on the region and quantified the number of blood vessels.RESULTS:No significant differences in the number of blood vessels between the left and right patellar tendons were found within groups. Additionally, no significant differences in the number of blood vessels in the left patellar tendons were found between groups.CONCLUSIONS:The application of high-energy extracorporeal shockwave therapy did not cause a change in vascularization in the patellar tendon in rabbits.
OBJETIVO: Avaliar os efeitos da aplicação de ondas de choque na placa de crescimento de coelhos, variando a energia e o número de ondas aplicadas. MATERIAL E MÉTODOS: Foram estudados 30 coelhos com idade de cinco semanas divididos em seis grupos. O aparelho gerador de ondas de choque foi o OSSATRON (HMT). Os animais foram submetidos a aplicações na placa de crescimento proximal da tíbia esquerda, variando a intensidade (14, 21,28kV) e o número de ondas (1000 e 4000). A tíbia direita foi usada como controle. Os animais foram sacrificados após seis semanas e sua avaliação foi feita por meio da mensuração do comprimento das tíbias e pela análise microscópica da placa de crescimento. Os resultados de mensuração da área da placa e do comprimento das tíbias foram comparados com o grupo controle e entre os grupos, utilizando-se estudos estatísticos. RESULTADOS: Neste estudo não ocorreram alterações estatisticamente significantes nas medidas da área da placa ou do comprimento da tíbia após a aplicação de ondas de choque em nenhum dos grupos. Foi observada tendência de ocorrer uma diferença de crescimento das tíbias entre os grupos submetidos a 1.000 impulsos e os grupos submetidos a 4.000 impulsos, mas esta diferença não foi considerada significante. Foi observada também a formação de hematoma no local da aplicação nos grupos que receberam ondas com maior energia, evoluindo com regressão espontânea sem necessidade de cuidados especiais. CONCLUSÕES: A aplicação de ondas de choque na região proximal da tíbia de coelhos imaturos, nas formas descritas, não teve nenhum efeito na placa de crescimento, de acordo com os métodos de avaliação usados neste estudo.
Desde dezembro de 2019 houve um alerta para uma pneumonia viral identificada como COVID-19 causado pelo vírus Sars-Cov-2. Essa doença atingiu vários países e infectou milhões de pessoas. As consequências no longo prazo dessa infecção ainda estão sendo observadas e as doenças pré-existentes ou adquiridas podem agravar o status de saúde da população mundial. Em todos os casos, há inúmeros questionamentos da necessidade de acompanhamento, assistência e reabilitação. Inicialmente, o foco mundial esteve na contingência da pandemia e na sustentação de uma rede de serviços de urgência e emergência, além da suficiência de leitos de enfermaria e cuidados intensivos. Dando sequência nesse processo, um aspecto importante a ser estudado e planejado é a construção de uma jornada do paciente pós COVID-19. Diante disso, foi realizada uma pesquisa qualitativa aplicando o método de entrevista em profundidade junto a líderes da área de saúde no Brasil para compreender possíveis formas de viabilizar esta condição de aprimoramento da jornada do paciente. Baseado nesta pesquisa observou-se que a coordenação do cuidado assistencial pode ser um excelente caminho para redefinição da jornada dos pacientes infectados, apoiado por ferramentas tecnológicas que vão desde teletriagens e teleorientações, passando por autocuidado apoiado e interoperabilidade de dados de saúde. O cuidado coordenado promove entregas mais qualificadas em saúde, com desfechos baseados em valor como novo pilar na saúde suplementar e nas políticas públicas de saúde, que incluem, ainda, a revisão na remuneração dos serviços prestados. O artigo também discorre sobre os principais conceitos correlatos entre a coordenação do cuidado e suas interações com a aplicação de soluções baseadas em saúde digital e cuidado híbrido, além de destacar o papel da ciência cidadã na construção de uma nova jornada do paciente. Baseada nesta premissa procura-se constituir argumentos junto às lideranças da área de saúde para que possam refletir e promover uma revisão nos processos relacionados à jornada do paciente pós-pandemia.Palavras-chave: Estratégias de eSaúde, Políticas de eSaúde, Atenção Primária à Saúde, Assistência ao Paciente. ABSTRACTSince December 2019 there has been an alert for viral pneumonia identified as COVID-19 caused by the Sars-Cov-2 virus. This disease affected several countries and infected millions of people. The long-term consequences of this infection are still being observed and pre-existing or acquired diseases can worsen the health status of the world population. In all cases, there are numerous questions about the need for follow-up, assistance, and rehabilitation. Initially, the global focus was on the contingency of the pandemic and the support of a network of urgent and emergency services, in addition to the sufficiency of infirmary and intensive care beds. Following this process, an important aspect to be studied and planned is the construction of a post-COVID-19 patient journey. Therefore, a qualitative research was carried out applying the method of in-depth interview with health leaders in Brazil to understand possible ways to make this condition of improving the patient's journey feasible. Based on this research, he observed that the coordination of care can be an excellent way to redefine the journey of infected patients, supported by technological tools ranging from tele-screening and tele-orientations, through supported self-care and interoperability of health data. Coordinated care promotes more qualified deliveries in health, with value-based outcomes as a new pillar in supplementary health and public health policies, which also include a review of the remuneration for services provided. The article also discusses the main correlated concepts between the coordination of care and its interactions with the application of solutions based on digital health and hybrid care, in addition to highlighting the role of citizen science in building a new patient journey. Based on this premise, we seek to constitute arguments with health leaders so that they can reflect and promote a review of the processes related to the post-pandemic patient's journey.Keywords: eHealth Strategies, eHealth Policies, Primary Health Care, Patient Care.
