Purpose: Use of circulating tumor DNA for diagnosis is limited regarding the low number of target molecules in early stage tumors. HPV-associated carcinomas represent a privileged model using circulating viral DNA (ctHPV DNA) as a tumor marker. However, the plurality of HPV genotypes represents a challenge. The NGSbased CaptHPV approach is able to characterize any HPV DNA sequence. To assess the ability of this method to establish the diagnosis of HPV-associated cancer via a blood sample, we analyzed ctHPV DNA in HPV-positive or HPV-negative carcinomas.Experimental Design: Patients ( 135) from France and Senegal with carcinoma developed in the uterine cervix (74), oropharynx (25), oral cavity (19), anus (12), and vulva (5) were prospectively registered. Matched tumor tissue and blood samples (10 ml) were taken before treatment and independently analyzed using the CaptHPV method.Results: HPV prevalence in tumors was 60.0% (81/135) (15 different genotypes).Viral analysis of plasmas compared to tumors was available for 134 patients. In the group of 80 patients with HPV-positive tumors, 77 were also positive in plasma (sensitivity 95.0%); in the group of 54 patients with HPV-negative tumors, one was positive in plasma (specificity 98.1%). In most cases, the complete HPV pattern observed in tumors could be established from the analysis of ctHPV DNA. Conclusions:In patients with carcinoma associated with any HPV genotype, a complete viral genome characterization can be obtained via the analysis of a standard blood sample. This should favor the development of non-invasive diagnostic tests providing the identification of personalized tumor markers.
INTRODUCCIÓNLa enfermedad celíaca es una enteropatía crónica que ocurre en sujetos genéticamente predispuestos consistente en una respuesta inmunológica anormal a la fracción de gliadina del gluten, una proteína de los cereales que se encuentra en el trigo, el centeno, la cebada y en la avena. La gliadina actúa como un antígeno que promueve la agregación de linfocitos T en la mucosa intestinal provocando una atrofia de las vellosidades intestinales, responsable de la aparición de un síndrome de malabsorción (1). La respuesta inmune celular frente a la gliadina es el episodio primario de la lesión intestinal, mientras que la inmunidad humoral tiene un papel secundario como amplificadora de la respuesta inmune (2).Sólo un tercio de los casos se diagnostican en la infancia (1). La prevalencia en Europa oscila entre 1/300 y 1/1000 habitantes y ha aumentado mucho en las dos últimas décadas por el incremento en el diagnóstico de formas subclínicas (3). En el varón se suele diagnosticar entre la quinta y sexta décadas de la vida y en la mujer entre la cuarta y quinta décadas. La proporción varón-mujer es de 1:1 en la infancia y en la senectud y de 1:2 en la edad adulta. La prevalencia en mujeres es mayor probablemente porque la enfermedad se diagnostica más en la mujer al ser superior la demanda metabólica durante la edad fértil, y los déficits de hierro y calcio son más llamativos (4).La forma de presentación de la enfermedad celíaca varía de modo notable de un paciente a otro pudiendo ser clínica-mente evidente (incluyendo formas mono u oligosintomáti-cas), subclínica y latente. Más de un 50% de los pacientes celíacos adultos no presentan síntomas intestinales, siendo más frecuentes las formas oligosintomáticas y subclínicas (5), lo que dificulta su diagnóstico en el adulto.35 [0212-7199 (2004) RESUMENLa enfermedad celíaca es una patología crónica del intestino delgado, causada por una intolerancia al gluten, en la que hay una lesión característica e inespecífica que conlleva una malabsorción de nutrientes y vitaminas. En el adulto es una entidad infradiagnosticada debido a sus formas atípicas. Es fundamental realizar un diagnóstico precoz, pues la dieta exenta de gluten previene las complicaciones a largo plazo como el linfoma T intestinal y otras neoplasias digestivas, disminuyendo la mortalidad.Presentamos un caso de enfermedad celíaca del adulto oligosintomá-tica en una paciente con antecedente de anemia ferropénica crónica y menorragia, comentamos las alteraciones clínico-nutricionales y la evolución de la paciente.
